针药并用治疗糖尿病神经原性膀胱疗效观察

摘　　要：目的:探讨糖尿病神经原性膀胱的较佳治疗方法。方法:将患者随机分为治疗组与对照组,每组35例。对照组采用弥可保250μg,隔日1次肌肉注射治疗;治疗组采用弥可保250μg,隔日1次肌肉注射加针刺关元、中极、肾俞、次、会阳穴,每日治疗1次。比较两组治疗前及治疗后残余尿量;同时对两组症状改善有效率进行比较,并对患者进行心理、健康等护理。结果:两组治疗后,治疗组尿急、尿频、小便淋沥不尽、尿失禁及排尿困难改善率明显优于对照组,但排尿时间延长方面,两组比较差异无显著性意义。治疗后两组膀胱残余尿量较治疗前有明显改善,其差异有显著性意义;且治疗组较对照组改善明显,其差异亦有显著性意义。结论:弥可保加穴位针刺治疗糖尿病神经原性膀胱效果优于单纯弥可保治疗,在治疗的同时注意护理能明显提高患者的生活质量。Objective To probe into a better therapy for diabetic neurogenic bladder. Methods The patients were randomly divided into a treatment group and a control group, 35 cases in each group. The control group were treated with intramuscular injection of Methycobal 250 μg, once every other day; the treatment group were treated with intramuscular injection of Methycobal 250 μg, once every other day, and acupuncture at Guanyuan （CV 4）, Shenshu （BL 23）, Ciliao （BL 32）, Huiyang （BL 35）, once every day. The residual urine were compared before and after treatment in the two groups; the effective rate for improvement ofsymptoms were compared between the two groups. The mental and healthy nursing were conducted for the patients. Results After treatment, the improving rate for the urgency of urination, frequency of micturition, dribbling urination, urinary incontinence and dysuria in the treatment group was significantly better than that in the control group, but with no significant difference between the two groups in prolongation of urination time. After treatment, the residual urine in the bladder significantly improved in the two groups with more significantly improved in the treatment group than in the control group. Conclusion Methycobal plus acupuncture has a better result than the simple Methycobal for treatment of diabetic neurogenic bladder, and Strengthening nursing care in the treatment can significantly enhance life quality of the patient.
