摘　　要：目的探讨应用改良式引流装置行侧脑室和腰椎蛛网膜下腔持续交替引流术治疗脑室出血的疗效。方法将80例脑室出血病人随机分成两组,改良组(40例)采用改良式侧脑室和腰椎蛛网膜下腔持续交替引流术,传统组(40例)应用传统侧脑室引流结合腰椎穿刺术。结果改良组1例(2.5%)死亡,2例(5.0%)发生梗阻性脑积水;传统组5例(12.5%)死亡,6例(15.0%)发生梗阻性脑积水。改良组脑室出血清除速度大大快于传统组(X2=20.43,P<0.01);改良组疗效也明显优于传统组(X2=10.18,P<0.05)。结论改良式侧脑室和腰椎蛛网膜下腔持续交替引流术治疗脑室出血可明显降低病人的死亡率和伤残率,减少并发症和后遗症。Objective To study the curative effect to ventricle hemorrhage treated by continuous altemant drainage surgery on meliorative side ventricle and lower cavity of lumbar arachnoid by applying the improved drainage devices. Methods 80 cases of patients with ventricle hemorrhage were randomly divided into two groups. The meliorative group of 40 patients was applied with continuous ahernant drainage surgery on meliorative side ventricle and lower cavity of lumbar arachnoid, while the contrast group of 40 patients was applied with traditional drainage surgery on side ventricle combined with lumbar centesis. Results Meliorative group ： 1 case （ 2.5% ） died, and 2 cases （ 5.0% ） occurred with obstructive waterhead. Contrast group： 5 cases （12.5%） died, and 6 cases （15.0%） occurred with obstructive waterhead. The cleanup speed of ventricle hemorrhage for patients in the meliorative group is much faster than that in the contrast group （X^2 = 20.43, P 〈 0. 01 ）. The curative effect of the meliorative group is distinctly better than that of the contrast group （X^2 = 10. 18, P 〈 0. 05 ）. Conclusion Ventricle hemorrhage treated by continuous ahernant drainage surgery on meliorative side ventricle and lower cavity of lumbar arachnoid can distinctly reduce mortality and deformity rate, complication and sequelae, and improve patients＇ surviving and living ability.
