摘  要:目的根据岩骨段颈内动脉水平部(hpICA)的显微解剖特点,在颞下入路(STKA)中寻找并证实定位hpICA的新方法。方法观测20侧干颅骨的岩尖结构,用44侧福尔马林固定的成人尸头模拟STKA操作,测量有关定位参数并与三角形定位法进行比较。结果棘孔到蝶岩沟的距离为(3.26±0.47)mm(2.44~4.32mm)。平行四边形定位法均能准确定位hpICA,此法显露的hpICA面积和有效利用系数均非常显著地大于其他两种三角形定位法(P<0.01)。结论在STKA中平行四边形定位法是迄今为止最准确、方便、安全的hpICA定位法。Objective To find and confirm a new method of locating horizontal portion of the petrous segment of the carotid artery (hpICA) based on the microantomic feature of hpICA in subtemporal approach (STKA). Methods Morphometric analysis of petrous apex was performed on 10 (20 sides) dry adult skulls. 44 sides of perfused cadaveric heads were dissected to mimic the STKA and parameters about locating hpICA were measured and compared with triangular locating methods. Results The distance between foramen spinosum and petrosal sphenoid groove averaged ( 3.26 ± 0.47 )mm ( range, 2.44 - 4.32mm). Parallelogram method can locate hpICA accurately. The exposed area of hpICA and availability of this method were very significantly larger than that of the other two triangular methods ( P 〈 0.01 ). Conclusions Parallelogram method is so far the most accurate, convenient and safe method to locate and expose hpICA in STKA.
