经心包肺切除术302例分析

摘　　要：目的:提高中晚期中央型肺癌的手术切除率,降低肺结核手术难度。方法:对302例行心包内肺切除术患者的临床资料进行回顾性分析,对242例肺癌患者随访。结果:无手术死亡,围手术期内死亡3例,出现并发症150例,1、3和5年生存率分别为80.99%(196/242)、37.91%(69/182)和23.42%(26/111)。其中2例生存10年,1例生存14年。结论:中晚期肺癌或肺结核毁损患者心包外处理肺血管行肺切除困难时,切开心包处理肺血管,有利于安全地切除病肺,可明显提高中晚期肺癌切除率,延长生存期或改善生活质量。OBJECTIVE： To raise the respectability of lung cancer at the middle or late stage and decend the difficulty of the operation in pulmonary tuberculosis. METHODS： The clinical data of 302 cases of intrapericardial pneumonectomy were reviewed and analyzed. RESULTS： There was no operative mortality, 150 cases had postoperative complications and 3 cases died in perioperative period. The 1-,3-,5-year survival rates were 80.99% （196/242）, 37.91%（69/192）, 23.42% （26/111）. Two cases survived for 10 years, 1 case survived for 14 years. CONCLUSION： Intrapericardial pneumonectomy could raise respectability of lung cancer at the middle or late stage or the destroy pulmonary tuberculosis and improve survival rate so that provision of opportunity for further effective treatment is made possible.
