摘 要:目的:观察快速乙肝疫苗接种方案对标准方案无反应者的免疫效果.方法:健康受试者分快程序组(n=53)和时照组(n=48),分别按d0,7,14程序和标准程序(mo 0,1,6)三角肌内接种30μg CHO乙肝疫苗,于首针接种后1,3,7,12,24和36 mo分别查抗-HBs滴度等.结果:首针接种后1,3和7mo组1血清抗-HBs阳转率(64.2%,66.0%,61.5%)和抗-HBs保护率(54.7%,54.7%,47.3%)分别显著高于组2(0%,16.7%,52.1%;0%,4.1%,40.1%)(P<0.01):组1全程接种完成率和血清抗-HBs几何平均滴度(GMT)显著高于组2 (100% vs 60.4%;66.8,68.2 IU/L vs 0,5.4 IU/L,P<0.01).3针完成后1 mo无及弱反应者追加1针后两组均为保护反应(100%),组1和组2无反应者分别有54.5%和50.0%阳转.结论:d0,7,14方案接种方便;全程接种完成率高;抗-HBs阳转和GMT峰值出现早.对标准程序弱或无反应者接种4针可提高反应率.AIM: To analyze the effect of accelerated immunization schedule for those who have no responses to standard hepatitis B vaccination schedule. METHODS: Healthy subjects were divided into group A (n = 53) and B (n = 48), vaccinated by 30 μg (20 μg for those less than 12 years old) recombinant hepatitis B vaccine with the accelerated schedule (immunized on day 0, 7, 14) and standard schedule (immunized in month 0, 1, 6), respectively. The efficacy was detected 1, 3, 7, 12, 24 and 36 months after the first vaccination. RESULTS: The seroconversion rates and protection rates in group A were significantly higher than those in group B 1, 3 and 7 month after the first vaccination (64.2%, 66.0%, 61.5% vs 0%, 16.7%, 52.1%; 54.7%, 54.7%, 47.3% vs 0%, 4.1%, 40.1%; P 〈 0.01). The completion rate and geometric titer (GMT) of anti-HBs I and 3 month after the first dose in group A were also markedly higher than those in group B (100% vs 60.4%; 66.8, 68.2 IU/L vs 0, 5.4 IU/L, P 〈 0.01). One month after the third vaccination, another dose was given to those without responses or with weak responses, and the protective effect was observed in all the subjects of both groups. CONCLUSION: The accelerated immunization schedule is simple and completion rate, earlier effective, with a higher appearance of seroconversion and peak anti-HBs GMT. Booster dose is necessary for those who have low or no responses to the vaccine.
