表面麻醉联合眼球筋膜囊下麻醉在手法小切口白内障手术中的应用

摘　　要：目的评价表面麻醉联合眼球筋膜囊下浸润麻醉完成手法小切口白内障手术的优缺点。方法观察表面麻醉联合眼球筋膜囊下浸润麻醉完成手法小切口(非超声乳化)白内障198例(258眼)的麻醉效果及与麻醉有关副反应和并发症,并与表面麻醉联合球后阻滞麻醉151例(188眼)作对照分析。结果2种麻醉方法的效果均良好。眼球筋膜囊下浸润麻醉,眼球制动较差。球后阻滞麻醉,球后出血2眼(1.06%),一过性黑朦1眼(0.53%)。结论表面麻醉联合眼球筋膜囊下浸润麻醉行手法小切口白内障手术,用药量少,并发症少。Objective To evaluate the superiority and defect of topical anesthesia combined with sub-Tenon＇ s block in non- phacoemulsification small incision cataract surgery. Methods 258 eyes of 198 cases with non-phacoemulsification small incision cataract surgery under topical anesthesia combined with sub-Tenon＇ s block were observed; 188 eyes of 151 cases from June,2001 to January,2004 with non-phacoemulsification small incision cataract surgery under topical anesthesia combined with retrobulbar anesthesia were reviewed. The effect of anesthesia, side effect and complications associated with anesthesia were analysed. Results anesthesia methods were both effective. In sub-Tenon＇ s block, preventing the eyeball＇ s moving was more difficult. In retrobulbar anesthesia,2 cases （1.06%） with retrobulbar hemorrhage and 1 case （0.53%） with amaurosis fugax were found. Conclusions Topical anesthesia combined with sub-Tenon＇ s block in non-phacoemulsification small incision cataract surgery is simple, safe and effective. It needs little dosage and has few complications.
