摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者血清基质金属蛋白酶-9(MMP-9)及其抑制因子(TIMP-1)的变化与细胞黏附因子-1(ICAM-1)、血管内皮黏附因子-1(VCAM-1)之间的关系。方法应用酶联免疫吸附试验(ELISA)检测2005年1月至7月哈尔滨医科大学附属第一医院呼吸内科收治的58例COPD患者和同期来医院进行健康体检的30名正常人血清中MMP-9,TIMP-1、ICAM-1和VCAM-1的质量浓度,将MMP-9、TIMP-1和MMP-9/TIMP-1比值与ICAM-1、VCAM-1及第一秒用力呼气容积(FEV1)占预计值百分比(FEV1%)、1秒率(FEV1/FVC%)进行相关性分析。结果(1)COPD患者的MMP-9、TIMP-1、ICAM-1、VCAM-1血清质量浓度[(128.89±115.84),(228.28±107.13),(203.98±70.37),(352.98±117.73)μg/L]明显高于对照组[(30.65±18.43),(133.69±41.41),(148.35±23.77),(233.57±36.65)μg/L],差异均具有显著性意义(P均<0.01)。(2)COPD组血清MMP-9与ICAM-1、VCAM-1质量浓度呈正相关(P均<0.05);血清TIMP-1与ICAM-1、VCAM-1质量浓度呈正相关(P均<0.01);血清MMP-9、TIMP-1、ICAM-1、VCAM-1质量浓度与FEV1%及FEV1/FVC%均呈明显负相关(P均<0.01);MMP-9/TIMP-1比值与FEV1%及FEV1/FVC%均具有明显负相关(P均<0.05)。结论(1)COPD患者血中ICAM-1、VCAM-1增加与MMP-9、TIMP-1增加具有相关性,是COPD发病中基质金属蛋白酶降解细胞外基质的重要调节因素。(2)COPD患者血清MMP-9、TIMP-1明显增加,MMP-9/TIMP-1比值增加,与气流阻塞成负相关,MMP-9、TIMP-1是引起气流阻塞很重要的因素。Objective To investigate the ehange of MMP-9, TIMP-1, ICAM-1 and VCAM-1 expression in serum from patients with COPD,as well as the correlation between the expressions of these faetors and lung funetion. Methods Enzyme-linked Immunosorbent Assay( ELISA)was used to determine the levels of MMP-9,TIMP-1 ,ICAM-1 and VCAM-1 in serum from 58 patients with COPD and 30 samples from healthy donors in order to better understand the eorrelation between the expression of these faetors and airflow obstruetion. Results The serum levels of MMP-9,TIMP-1 ,ICAM-1 and VCAM-1 [ ( 128. 89 ±115. 84), (228.28 ± 107. 13 ), (203. 98 ± 70.37) and (352. 98 ± 117. 73 ) μg/L] from patients with COPD were statistieally and significantly higher than the eontrol group [ ( 30. 65 ± 18. 43 ), ( 133.69 ± 41.41 ), ( 148.35 ± 23. 77 ) and ( 233.57 ± 36. 65 ) μg/L], P 〈 0.01. There was positive eorrelation between MMP-9 and ICAM- 1 ,VCAM-1 respectively in COPD groups(P 〈0. 05)in term of serum level. Similar correlation was also shown between TIMP-1 and ICAM-1 .VCAM-1 (P 〈 0.01 ). In eontrast.there was a simaifieant inverse eorrelation between the levels ofMMP-9,TIMP-I,ICAM-1 and VCAM-1 in Serum and FEV1% (P 〈0. 01 ). This inverse correlation was also shown between MMP-9, TIMP-1, ICAM-1, VCAM-1 and FEV1/FVC% ( P 〈 0. 01 ). Furthermore, inverse relationships were observed between the ratio of MMP-9/TIMP-1 and FEV1 %, as well as MMP-9/TIMP-1 vs. FEV1/FVC% (P 〈 0. 05). Conclusion ( 1 ) Elevated serum ICAM-1 and VCAM-1 levels associated with the over-expression of MMP-9 and TIMP-1 suggest that these cellular adhesion molecules also play a certain role in the development of COPD. (2)There are significant elevations of MMP-9 and TIMP-1 in serum of patients with COPD, and increased ratio of MMP-9/TIMP-1 inversely correlates with airflow obstruction. These results suggest that MMP-9 and TIMP-1 might play an important role in the development of airway obstruction.
