摘  要:目的通过地塞米松对脂多糖(Lipopolysaccharides,LPS)诱导的大鼠急性肺损伤时炎症介质的影响作用,探讨其对肺脏的保护机制。方法健康雄性S-D大鼠随机分成A、B、C、D组,各组分别按1、2、4h三个时间点再设立3个亚组。A组经腹腔注射地塞米松;B组经腹腔注射LPS,C组在腹腔注射LPS前1h经腹腔注射地塞米松,D组为对照组。在到达各时间点后,经腹腔注射加倍剂量的硫喷妥钠处死,切开气管,并予0℃0.9%氯化钠注射液(normal saline,NS)作肺泡灌洗。采用酶联吸附免疫法(ELISA)测定肺泡灌洗液(bronchoalveokarlavage fluid,BALF)中的肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白介素-1β(iaterlrukin-Iβ,IL-1β)与巨噬细胞炎性蛋白-2(macrophafeinflamnatovy protein-2,MIP-2)。另外12只大鼠按上述分组法随机分成4组,每组3只,动物模型同上,作肺组织病理观察。结果在病理形态学上与D组比较,B组肺损伤严重。BALF中TNF-α、IL-1β、MIP-2在A、B、C、D组间比较差异均有统计学意义(F分别=15.51、47.37、19.94、134.27、65.44、57.29、18.44、85.31、89.13,P均<0.05)。B组与D组比较,TNF-α差异有统计学意义(t分别=5.37、8.69、6.43,P均<0.05)、IL-1β差异有统计学意义(t分别=7.25、9.77、11.51,P均<0.05)、MIP-2差异有统计学意义(t分别=6.84、12.38、15.52,P均<0.05);C组与D组比较,TNF-α差异有统计学意义(t值分别为2.43、2.67、1.98,P均<0.05)、IL-1β差异有统计学意义(t分别=3.58、4.15、4.81,P均<0.05)、MIP-2差异有统计学意义(t分别=1.87、3.73、7.55,P均<0.05);C组与B组比较,TNF-α差异有统计学意(t分别=3.15、4.19、4.03,P均<0.05)、IL-1β差异有统计学意(t分别=3.71、5.27、5.81,P均<0.05)、MIP-2差异有统计学意义(t分别=4.33、6.89、7.14,P均<0.05)。结论地塞米松可降低炎症介质水平,提示对肺脏具有保护作用。Objective To study the protective effects of dexamethasone on lipopolysaccharide(LPS) - induced lung injury in rats. Methods Seventy two S- D rats were randomly divided into 4 groups (groupA, B, C, D), and each group divided into 3 subgroups at 1, 2 and 4 hours. Group D was control, group A received DEX (1mg/kg) by intraperitoneally injection, group B received LPS (5mg/ kg) by intraperitoneally injection, and group C received DEX 1 hour before LPS by intraperitoneally injection. All the animals were killed with overdose of sodium pentathol at 1, 2 and 4 hours respectively. The bronchoalveolar lavage (BAL) was performed through a tracheal cannula with 8 ml ice - saline each rat, and the withdrawal rate of broncho alveolar lavage fluid (BALF) was more than 70%. The concentration of tumor necrosis factor -α (TNF - α), interleukin - 1β (IL—1β ), and macrophage inflammatory protein- 2 (MIP- 2) were monitored in BALF by ELISA. Another 12 rats were randomly divided into 4 groups, 3 rats in each group for histological study. Results Lung injury was serious in group B compared with group D. There were no significant differences in the concentration of TNF - α, IL—1β and MIP - 2 between group A and D (P 〉 0.05). These parameters were significantly higher in group B and C than that of group A and D ( P 〈 0.05), and increased more significantly in group B (P 〈 0.05) . The concentration of IL—1β and MIP - 2 were increasing as the time elongation, and peaked at 4 hours ; TNF - α peaked at 2 hours in these two groups . IL—1β , MIP - 2 and TNF - α were significantly lower in group C than that of group B ( P 〈 0.05) . Conclusion Dexamethasone could protect LPS - induced lung injury through inhibit inflammatory mediators.
