普贝生足月引产延长放置时间的研究

摘　　要：目的：探讨普贝生延长放置用于足月妊娠引产的临床效果及母婴安全性。方法：采用随机对照前瞻性研究方法,对正常单胎头位足月妊娠,具有引产指征而无使用前列腺素禁忌症的孕妇40例,随机分成常规放药组（放药时间≤12h）24例和延长放药组（放药时间〉12h）16例,阴道放入普贝生,观察用药至出现宫缩的时间、强度、频率,临产开始时间、分娩时间、阴道分娩率、催产素使用量、总产程、产后24h出血量和新生儿体重、Apgar评分及出现子宫过度刺激等情况的差异。结果：普贝生对各组促宫颈成熟的总有效率为80%,用药至临产的时间2.3~25h,平均（16.08±6.87）h;放药至分娩时间为6.5~34h,平均（21.58±7.29）h。引产总有效率,常规放药组为83.33%,延长放药组为100.00%,P〈0.05;其总产程、临产后催产素的用量常规放药组均多于延长放药组;阴道分娩率、产后出血量及新生儿Apgar评分两组无显著性差异。结论：普贝生对足月妊娠促宫颈成熟有效,引产成功率高,适当延长放药时间,仍然安全有效。Objective： To explore the clinical efficacy and safety of the mothers and babies whom be used the propess extend 12 hours. Methods： By randomized control prospective study, 40 cases of term monocyesis with cephalic presentation and indication of induced labor but no prostaglandin usage contraindications were divided into＇ two groups randomly. Medicine use routine group used propess in 12 hours, and prolong use group used propess extend 12 hours. The time, tense, fregnency of uterine unbraction , the time of labor starting , the time of labor, labor in vaginal, the application of oxytocin, the weight of neonatal , the Apgar score, the uterine hyperstimulation were compared between the two groups. Results： The efficicency of riper the cervix was 80%. The time of labor starting was 2.3 ～ 25 hours, average 16.08 ±6. 87 hours . The time of labor was 1.5 ～34 hours, avarsge 21.58 ± 1.29 hours. The total of labor induction was 83.33% in medicine use routinegroup and 100% in the prolong use group. The total labor time and the application of oxytocin were much less in the prolong use group. There was no significant difference between two groups in labor in veginal, postpartum hemorrhage and Apgar score. Conclusion： Propess is effectively to ripen the cervix and successful laor induction. Prolong the use properly is still safety and effec- tively.
