未明热中的Still病11例分析

摘　　要：成人Ｓｔｉｌ病近年来作为未明热的基础疾患引起人们高度的重视。近５年我院收治６８例内科未明热患者中，确诊为Ｓｔｉｌ病者１０例。临床均有发热及关节痛，有皮疹７例，肝脾肿大４例，咽痛３例，胸腔及心包积液各１例，１０例均有心悸感，２例示窦性心动过速，１例房室传导阻滞，１例频发室早，间质性肺炎１例，１０例血沉均增快达８３ｍｍ／ｈＣ反应蛋白阳性，白细胞增高，贫血、低蛋白血症及高ｒ—球蛋白血症１０例，ＣＨ５０升高者１０例，ｃ３升高者２例，类风湿因子和抗核抗体阳性各１例，均无损害。７例血清铁蛋白升高，平均为３１１μｇ／Ｌ，是病情活动的表现，可作为病情判断和治疗效果观察的可靠指标。治疗主张以非甾体消炎药为主，２例好转，全身症状重者合并小至中等量激素治疗，７例中痊愈１例，好转５例，关节炎持续时间长者可酌情考虑免疫调节剂。经验告诉我们大剂量应用甲基强地松龙，易形成激素依赖性及合并致死性的严重感染。$$$$ Song Renling,et al Department of Clinical Laboratory,First Affiliated hospital,Dalian Medical UniversityAbstract We study the relation of the progression of disease and the change of serum IgE level and peripheral blood T cell subset in 45 patients with SLE.Result shows:Serum IgE level (286 5±143 6IU/ml)With SLE in active period is remarkably higher than those of patients in inactive period.(88 5±50 2IU/ml)and those of normal control objects(86 5±53 6IU/ml).At the same time,comparison of the results of Tcell subset in inactive period.CD4 4increases and CD8 +deseases.This article demonstrates:the study of serun IgE change in patients with SLE is helpful for the understanding of disease progression and for the clinical therapy.
