嘉兴市血吸虫病传播阻断后疫情监测11年及效果评价

摘　　要：目的掌握1995-2006年浙江省嘉兴市达到血吸虫病传播阻断标准后流行因素的动态变化,为制订防治策略提供科学依据。方法采用机环结合查螺法实行普查、轮查、设监测点等方法查螺;用血清学方法对当地遗漏传染源以及外来流动人员、外来的耕牛(菜牛)开展查病。结果在4个县区38个村查到226个有钉螺条块(次),面积50.46×104m2,捕获并解剖钉螺57620只,无阳性钉螺。查病74万人,查到输入病例3例,当地无病例。结论本地区血吸虫病疫情稳定,但存在残存钉螺。今后监测的重点应为未经环境改造的历史螺区及其延伸环境、来自疫区的流动人员以及医疗单位收治的疑似病例。Objective This study was designed to know well the dynamic changes of schistosomiasis prevalent factors after the criteria of transmission interruption had been met from 1995 to 2006 in Jiaxing City and provide scientific basis for formulating prevention and control strategies. Methods The snail situation was surveyed through general, round, monitoring-site surveys, etc. Using the method of mechanical combining environmental frame-setting sampling, schistosomiasis cases were serologically detected from missed sources of infection, floating population and floating cattle. Results 226 marshlands with snails were detected from 38 villages in 4 counties/cities covering an area of 50.46×10^4 m^2, 57 620 snails were captured and dissected and no positive snails were discovered. Three imported schistosomiasis cases were detected form 740 000 people surveyed without local infections. Conclusion The situation of schistosomiasis in this city is stable but there are residual snails. The surveillance emphasis should be put on the floating population from epidemic zones, history snail areas without environmental modification and their extensions, and suspicious cases.
