过敏性紫癜急性期血浆白细胞介素-15水平的变化及意义

摘　　要：目的探讨过敏性紫癜(HSP)患儿急性期血浆白细胞介素-15(IL-15)水平的变化及其意义。方法以25例健康儿童为正常对照组,对61例HSP患儿急性期和缓解期血浆IL-15水平进行检测,并对不同临床类型间急性期血浆IL-15水平进行比较。血浆IL-15水平用双抗体夹心ELISA法检测。结果①HSP组急性期血浆IL-15水平为(17.5±10.2)ng/L,显著高于HSP组缓解期[(5.7±4.3)ng/L]和正常对照组[(4.2±3.5)ng/L],差异有极显著性意义(P<0.01),而HSP缓解期与正常对照组间差异无显著性意义(P>0.05);②急性期有、无肾损害者血浆IL-15水平分别为(29.1±16.5)ng/L和(10.5±7.4)ng/L,均极显著高于正常对照组(P均<0.01);急性期有肾损害组血浆IL-15水平极显著高于无肾损害组(P<0.01)。结论IL-15在HSP的发病机制特别是紫癜性肾炎的发生过程中起重要作用。Objective To probe the changes and clinical significances of interleukin-15（IL-15） in children with henochschoenlein purpura（HSP）. Methods Plasma IL-15 levels were detected in 61 children with HSP and 25 normal children by ELISA. The level of plasma IL-15 in different clinical categories of HSP were compared. Results （1）Plasma IL-15 levels in children with HSP in acute phase[ （17.5 ± 10. 2）ng/L] were significantly higher than those in remission phase[ （5. 7± 4. 3）ng/L] and healthy controls[ （4. 2± 3. 5）ng/L] （P〈0. 01 ）;Plasma IL-15 levels were no significantly different between healthy controls and remission phase of HSP in children（P^0. 05）. （2）Both plasma IL-15 levels in acute phase of HSP with and without renal involvement[（29. 1 ± 16. 5）ng/L, （10. 5 ± 7. 4）ng/L] were significantly higher than those in healthy controls（P^0.01 ）;The plasma IL-15 levels in patients with renal involvement were significantly higher than those of without renal involvement（P〈0. 01）. Concluian These results suggested that IL-15 play a crucial role in pathogenesis of HSP and HSP nephritis.
