摘  要:目的探讨联合检测抗dsDNA、尿微量白蛋白、血细胞变化及D-二聚体对系统性红斑狼疮(SLE)的诊断价值。方法96例初诊SLE患者及36名健康体检者全血,用全自动血液细胞分析仪检测血细胞计数,用胶体金抗体免疫渗滤法检测血浆D-二聚体水平,用免疫散射比浊法检测尿微量白蛋白,用胶体金斑点渗滤法检测血清抗dsDNA抗体,并进行分析比较。结果SLE特异性指标抗dsDNA阳性率仅为42.7%,而血细胞异常率较高达70.8%,早期肾损害的指标尿微量白蛋白阳性率为65.6%,D-二聚体阳性率为58.3%;四者联合检测阳性率提高至90.6%。结论联合检测抗dsDNA、尿微量白蛋白、血细胞变化及D-二聚体对SLE的诊断具有重要意义,可提高对SLE的检测敏感性。Objective To evaluate the significance of combined determination of anti-dsDNA, urine microalbumin (MA), blood cells and D-dimer in the diagnosis of systemic lupus erythematosus (SLE). Methods In 96 cases of SLE and 36 cases of healthy people, serum anti-dsDNA and plasma D-dimer were detected by colloidal gold method, urine MA by immune dispersion scatter turbidity method and blood cells were counted by automatic blood cells analyzer. Results The positive rate of anti-dsDNA, low blood cells, urine MA, and D-dimer in patients with SLE was 42. 7%, 70. 8% ,. 65.6%, and 58. 3% respectively, while after combined determination of anti-dsDNA, low blood cells, urine MA, and D-dimer, the positive rate was 90. 6%. Conclusion The combined determination of anti-dsDNA, low blood cells, urine MA, and D-dimer may improve the sensitivity of laboratory diagnosis of SLE.
