摘 要:目的研究卡维地洛对长期快速起搏实验犬心房有效不应期(AERP)和心房电重构的影响。方法27只犬随机分为3组,起搏组:采用快速起博心房的方法建立房颤模型(起搏频率400次/min),起搏器置入前及起搏6周后分别行电生理检查,以2个基本周长(S1=300、200ms)分别标测AERP;起搏加药物组:起搏器置入及电生理检查同起搏组,起搏器置入前3d至起搏6周,每日给予卡维地洛12.5mg,2次/d口服;对照组:实验犬未置入起搏器,仅于相应的时间行电生理检查。结果起搏组起搏6周后较起搏前AERP明显缩短(P<0.001);起搏加药物组起搏6周后与起搏前比较,AERP无明显变化(P>0.05);对照组6周前、后所测AERP差异无统计学意义(P>0.05)。结论实验犬长期快速起搏可致AERP缩短,卡维地洛可明显抑制长期快速起搏实验犬AERP的缩短和AERP频率适应性丧失,抑制心房电重构。Objective To investigate the effects of carvedilol on atrial effective refractory period (AERP) and atrial dectrical remodeling in canine atrium with long-time rapid pacing. Methods Twenty-seven dogs were randomly divided into 3 groups. In pacing group (n=9), each dog was implanted with permanent pacemaker (pacing at 400 bpm), Electrophysiological study was performed before and after 6 weeks of rapid pacing, respectively. AERP was measured at two different basic cycle lengths (BCLs) (S1=300,200 ms). In pacing and drug group (n=9), each dog was given carvedilol 12.5 rag bid three days before the pacemaker was implanted and continued through the study. The pacing procedure was as sarae as the pacing group. AERP was measured before and after 6 weeks of rapid atrial pacing. In control group(n=9), no pacemakers were implanted, but all dogs were given electrophysiological study in the corresponding time with the experimental groups. Results There was a significant decrease in AERP as a result of 6 weeks rapid atrial pacing compared with baseline in pacing group (P〈 0.001). Inpacing and drug group, there were no differences in AERP before and after 6 weeks rapid pacing (P〉 0.05). In control group, no differences of AERP were found in the different electrophysiological study time (P〉 0.05). Conclusion Long-term rapid atrial pacing results in the shortening of AERP in dogs. Carvedilol may prevent the shortening of AERP and the lose of the physiological rate adaptaion of AERP, and prevent the atrial electrical remodeling in dogs with long-term rapid atrial pacing.
