2型糖尿病合并大血管病变患者血浆脂联素水平变化的研究

摘　　要：目的探讨2型糖尿病合并大血管病变患者血浆脂联素(APN)水平的变化及意义。方法随机选取2型糖尿病合并大血管病变患者34例(A组)、单纯2型糖尿病患者34例(B组)、正常体健者20例(C组),分别检测3组患者的体质量指数(BMI)、血压、空腹血糖(FBG)、空腹胰岛素(FINS)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、血浆纤溶酶原激活剂抑制剂-1(PAI-1)水平,比较APN水平的变化,并与其他因素进行相关分析及回归分析。结果A组和B组血浆APN水平均低于C组(P<0.01),A组低于B组(P<0.01)。多元线性相关分析显示,A组APN与收缩压、BMI、FBG、FINS、TG水平呈负相关,与血浆PAI-1水平亦呈负相关。结论2型糖尿病患者中血浆APN水平显著降低,合并大血管病变者更低,脂联素与2型糖尿病及大血管病变之间存在一定相关性。Objective It is to explore the change of plasma adiponectin （APN） level and its significance in patients with 2 type diabetes complicating with macroangiopathy. Methods 34 cases of patients with 2 type diabetes complicating with rnacroangiopathy （group A）, 34 cases of ones with 2 type diabetes only （group B） and 20 cases of healthy people （group C） were randomly chosen and determined their levels of BMI, blood pressure, FBG, FINS, TC, TG, LDL - C and PAI- 1. The changes of APN level in the three groups were compared and its regression analysis and correlation analysis with other factors were done. Results The level of APN in group A and group B was lower than that in group C （P〈0.01）, and the level in group A was lower than that in group B （P 〈 0.01 ）. Multielement linearity correlation analysis showed that the level of APN in group A had a negative relationship with systolic pressure, BMI, FINS, FBG, TG and PAI- 1. Conclusion The level of APN significantly decreases in patients with 2 type diabetes, especially in the ones complicating with macroangiopathy. APN has some correlated relationship with 2 type diabetes and macroangiopathy.
