摘 要:为了研究硬膜外麻醉对肾移植受者血流动力学的影响和意义,根据血压高低将肾移植患者分为3组,采用全自动无创血压监测仪测定患者麻醉前、手水平面固定后的基础动脉收缩压(SBP)并进行比较。结果表明,SBP<22.7kPa(1kPa=7.5mmHg)者,血压变化不明显;SBP为22.7~25.3kPa者,5例中有3例血压下降,但下降幅度均<4kPa;SBP>25.3kPa者,所有病例均有明显的血压下降。由此认为,肾移植术中硬膜外麻醉对受者循环的影响与其术前的基础血压在接相关,尤以SBP>25.3kPa时更为明显。In order to study the effects of epldural anesthesia on hemodynamics in renal transplantation, 18 patients with different blood pressure were dlvided into 3 groups. Systolic blood pressure (SBP) was measured by non-invasive automatic tonometer be fore and after anesthesia.No obvious change in SBP was found in patients with SBP<22. 7 kPa; a mild SBP decrease (<4kPa) was noted in patients with SBP ranging between 22. 7 ~ 25. 3 kPa and an obvious SBP decrease was found in all the patients with SBP>25. 3 kPa. Our results suggested that the hemodynamics in renal transplantation patients was closely related to the basic SBP, especially when SBP was greater than 25. 3 kPa.
