腰围变化对代谢综合征综合干预效果的影响

摘　　要：目的了解综合干预后腰围的变化对代谢综合征(MS)控制情况的影响。方法对490例糖尿病、高血压、MS患者多种危险因素进行综合干预后,根据干预前后腰围的变化,排除64例腰围无变化者,将有变化的患者分为腰围下降组(212例)和腰围上升组(214例),对2组患者相关代谢指标的改善情况进行比较。结果腰围下降组胰岛素抵抗指数(HOMA-IR)和高敏C反应蛋白(hs-CRP)水平显著下降(P<0.01),MS各组分达标率升高,危险因素显著减少;而腰围上升组各项代谢指标改善不显著。hs-CRP、HOMA-IR、舒张压、甘油三酯的变化值与腰围的变化值呈正相关(P<0.05)。结论随着综合干预后腰围的下降,胰岛素抵抗和炎性反应减轻,MS各组分得到改善,危险因素减少。Objective To study the effect of change of waist circumference（WC） on the result of comprehensive intervention of metabolic syndrome（MS）. Methods The metabolic indexes of 490 patients with diabetes, hypertension or MS before and after comprehensive intervention were analyzed retrospectively. The patients were divided into WC-incresed group （ n =214） and WC-lowered （ n =212） group,and then the metabolic indexes of the two groups were compared. Results After comprehensive intervention,in WC-lowered group, the insulin resistance（IR） and hypersensitive C-reactive protein（hs-CRP） decreased significantly（ P 〈0.01）, the rate of control of each MS component increased, and risk factors were reduced significantly;whereas these indexes in WC-increased group were not significantly improved. The changes of hs-CRP , IR, diastolic blood pressure and triglycerides were positively correlated to the change of WC（ P 〈0.05）. Conclusion With decreasing of WC, IR and inflammation were relieved,and the risk factors were de- creased or became controlled.
