宫颈胰岛素样生长因子结合蛋白-1与宫颈超声测定学预测分娩的研究

摘　　要：目的探讨宫颈分泌物中胰岛素样生长因子结合蛋白-1(IGFBP-1)与阴道B超评价宫颈成熟度的价值以及两者相结合预测分娩时机的价值。方法选择120例妊娠37～40周的孕妇,先用免疫层析法测定宫颈分泌物中的高磷酸化IGFBP-1,之后,经阴道B超测定宫颈长度、宫颈内口宽度、羊膜囊嵌入宫颈内影像(HM),并计算出宫颈超声评分。每周测定一次,直至自然临产。结果94例孕妇最终被纳入研究之列。IGFBP-1阳性者自然临产时间短于阴性者;宫颈超声评分高者自然临产时间短于低评分者,差异显著,两者相结合预测自然临产时间的回归方程为:距自然临产时间(天)=3.423-1.001×宫颈超声评分+3.875×宫颈阴道分泌物IGFBP-1+0.142×年龄。结论宫颈分泌物中IGFBP-1和宫颈超声评分均能反映宫颈成熟度,两者相结合能更全面、客观评价宫颈成熟度,预测分娩时机。Objective To discuss the value of insulin like growth factor binding protein-1（IGFBP-1） in cervix secretion and cervical ripening score in the prediction of cervical ripness. And then to estimate,the chance of delivery by testing both of them. Methods 120 normal pregnancy women of 37-40 gestation weeks received test by using the method of immunochromatography on their phosphorylated isoforms of IGFBP01 in cervix and trans-vaginal ultrasound to measure the cervical length, cervical internal os widith, and image of amniotic sac inserted in cervix which were totally assessed. The pregnancy women at term reseived these tests once a week until natural parturient. Results 94 cases of normal pregnancy women were suitable for the study in the end. The parturient time was shorter in those pregnancy women whose IGFBP-1 in cervix was positive, longer in those whose IGFBP-1 in cervix was negative. The higher of cervical ripening score, the shorter of parturient time in pregnancy women. The regression equation of natural parturient time is 3.423-1. 001×cervical ripening score＋3. 875×IGFBP-1 in cervix ＋0. 142×age. Conclusions Both IGFBP-1 and cervical ripening score can reflect the ripness of cervix. It is more objectively and thoroughly to predict the chance of delivery while testing both IGFBP-1 in cervix and cervical ripening score.
