超声引导活检术在肺部包块介入诊断中的应用

摘　　要：目的探讨超声引导下经皮肺肿物穿刺活检方法及诊断价值。方法在B超引导下,采用快速自动活检枪配套切割式活检针对43例肺部肿块术前定位并确定穿刺路线。结果43例确诊,肺癌32例,结核3例,炎性假瘤3例,一般炎症4例,曲霉菌病1例。术后一例出现少量气胸,一例出现少许血丝痰,均未做特殊处理而恢复。结论超声引导下肺肿块穿刺活检,是简便、价廉、成功率高、并发症少的组织学取材方法。Objective To evaluate the diagnostic accuracy and the clinical application of ultrasound（US）guided percutaneous automatic biopsy of lung mass.Methods Percutaneous automatic biopsy was performed under US-guidance in 43 patients with lung masses,using a core needle with automatic biopsy device.The skin puncture site and needle route were carefully considered before biopsy.The post-procedure complications（including pneumothorax and bleeding）were noted.Results The procedures were successful in all patients and the histological reports were lung cancer in 32 patients,inflammatory nodules in 3 and tuberculosis in 3 patients respectively,4 with inflammation,1 pulmonary aspergillusis.Conclusion US-guided percutaneous automatic biopsy is a safe,reliable and economic method for the diagnosis of peripheral lung masses.
