实时三维超声分析扩张型心肌病心肌节段运动的差异性

摘　　要：目的用实时三维超声(RT-3DE)的左室局部射血分数(REF)探讨扩张型心肌病(DCM)患者左室心肌节段收缩运动的差异性。方法应用RT-3DE对39例DCM患者的左室节段运动进行定量分析。根据16条左室局部容积曲线,计算REF,将收缩期室壁无运动及运动减弱的左室节段,视作运动异常节段;将REF>30%的心肌节段,作为左室代偿节段。结果在DCM组624个心肌节段中,该类左室代偿节段占28.21%。左室代偿运动节段百分率与EF、SV显著相关(r值分别为-0.69与-0.52,P<0.001)。结论RT-3DE技术所测REF能够反映DCM代偿节段的数目与分布范围。笼统将DCM的心肌运动"普遍减低"作为鉴别诊断该病的心脏超声特征是值得商榷的。Objective To explore differences of left dilated cardiomyopathy （ DCM ） using real time Methods Quantitative analysis of LV segmental function ventricular （LV）systolic segmental motion in patients with three-dimensional echaocardiography （ RT-3DE ） was made in 39 patients with DCM on RT-3DE according to 16 curves of LV regional volume and regional ejection fraction （REF） . Segments of akinesis or hypokinesis were considered as abnormal and the segments with REF 〉 30% were defined as compensated motion. Results Compensated segments were observed in every case with DCM. In total of 624 segments in the DCM group, the compensated segments was 28.21%. Percentage of compensated segments in patients with DCM was closely related to ejection fraction and stroke volume （r = -0.69 and -0.52 respectively, P 〈 0.001 ） . Conclusions Compensated segments were existed in patients with DCM. Therefore, consideration of universal decrease of segmental motion in LV as one of the echocardiagraphic features of DCM is worthy to be recommended.
