摘 要:目的评价左旋布比卡因和布比卡因对体外人中性粒细胞(PMN)黏附分子CD11b表达的影响。方法健康志愿者12名,男性、女性各6名,体重55~68kg,年龄20~25岁。每名健康志愿者取8份(每份0.2ml)血样,分别随机接受不同处理:0.2ml血样+10μl磷酸盐缓冲液,37℃孵育60 min(对照组,Ⅰ组);Ⅱ组、Ⅲ组在Ⅰ组基础上分别加入血小板活化因子(PAF)、趋化寡肽(FMLP);Ⅳ组在Ⅰ组基础上加PAF,37℃孵育5min后加FMLP;Ⅴa组、Ⅴb组和Ⅴc组在Ⅳ组基础上加入布比卡因10μl,浓度分别为1×10^-6、1×10^-5和1×10^-4mol/L;Ⅵ组在Ⅳ组基础上加入左旋布比卡因10μl(终浓度为1×10^-4mol/L)。FMLP和PAF终浓度均为1×10^-6mol/L。测定PMN黏附分子CD11b的表达。结果与Ⅰ组比较,Ⅱ组CD11b表达差异无统计学意义(P>0.05),Ⅲ组、Ⅳ组CD11b表达上调;Ⅳ组高于Ⅲ组(P<0.01)。与Ⅳ组和Ⅴa组比较,Ⅴb组、Ⅴc组和Ⅵ组CD11b表达下调;Ⅴc组低于Ⅴb组(P<0.05或0.01);Ⅴc组与Ⅵ组差异无统计学意义(P>0.05)。结论布比卡因可抑制PAF和FMLP诱导PMN黏附分子CD11b的表达,且呈浓度依赖性;左旋布比卡因和布比卡因对PMN黏附分子CD11b表达的抑制作用无差异。Objective To evaluate the effects of levobupivacaine and racemic bupivacaine on CD11b expression in human neutrophils(PMNs). Methods Twelve healthy volunteers (6 male, 6 female) aged 20-25 yrs weighing 55-68 kg were recruited in this study. Venous blood samples (2 ml) were obtained from each volunteer and heparinized with heparin 20 U. Each blood sample was divided into 8 groups: group Ⅰ control; group Ⅱ platelet activating factor (PAF) 1 × 10^-6 mol/L was added to 0.2 ml of blood; group Ⅲ N-formyl-methione-L-phenylalamine (FMLP) 1 × 10^-6 mol/L was added to 0.2 ml of blood; group Ⅳ PAF+ FMLP; group Ⅴ PAF + FMLP + racemic bupivacaine 1 × 10^-6 (Va), 1 × 10^-5 (Vb) or 1 × 10^-4 (Vc) mol/L and group Ⅵ PAF + FMLP + levobupivacaine 10^-4 mol/L. After being incubated for 20 min WBCs were isolated and CD11b expression in PMNs was determined by flow cytometry.Results The expression of CD11b in PMNs was significantly upregulated in group Ⅲ and Ⅳ as compared to control group. There was no significant difference in CD11b expression in PMNs between group Ⅰ and Ⅱ . CD11b expression in PMNs in group Ⅴb and Ⅴc was significantly decreased as compared to group Ⅵ and Ⅴa. Conclusion Racemic bupivacaine can inhibit the expression of CD11b in PAF-primed and FMLP-activated PMNs in a concentration dependent manner. There is no significant difference in the inhibitory effect on CD11b expression in PMNs between levobupivacaine and racemic bupivacaine.
