胰腺闭合性损伤合并主胰管断裂的诊断与处理

摘　　要：目的探讨胰腺闭合性损伤合并主胰管断裂的诊断和外科处理方法。方法回顾性分析近10年的17例胰腺损伤病例资料,就胰腺损伤的特点、诊断方法及手术处理方式进行分析。术前术中正确运用各种方法进行诊断,行AAST损伤分级,确定胰腺损伤的严重程度,采用不同的手术术式进行处理。结果术后并发胰漏4例(23.53%),经引流后治愈,引流时间最短12d。最长2个月;腹内感染2例(11.77%),切口感染4例(23.53%),未发生手术死亡。结论及时正确的手术处理胰管损伤有助于减少并发症的发生,提高治愈率。Objective To investigate the diagnosis and treatment of the closed pancreas damage complicated with main pancreas pipe break. Methods 17 in hospital cases during the recent 10 years were retrospectively analyzed and the feature, diagnosis and operation were analyzed. Before and in the process of operation, these cases were graded by AAST to determine the degree of pancreas damage and to treat with different surgical methods. Resuits After operation,there were 4 cases of comphcated leak （23.5%） ,cured after drainage within 12 to 60 days; 2 cases of abdomen infection （ 11.77 % ） ,4 cases of cut infection （ 23.53 % ）. No surgical death occured. Conclusion The prompt operation on pancreas damage is helpful for reducing the complications and improving the healing rate.
