单病种基本医疗限价可扩展服务模式的实践与思考

摘　　要：目的:探讨单病种基本医疗限价可扩展服务模式在规范医疗行为及控制医疗费用不合理增长方面的作用。方法:选择我院三种手术治疗为主的常见疾病,制定并实施以临床路径为基础的单病种基本医疗限价可扩展服务模式,记录分析服务过程中出现的变异,并与未按此服务模式诊疗的患者进行对比分析。结果:同一疾病2006年按单病种基本医疗限价可扩展服务模式诊疗的患者,与2005年同期未按此服务模式诊疗患者比较,住院总费用、药费构成比及平均住院日明显降低,手术量明显增加。结论:单病种基本医疗限价可扩展服务模式,能满足不同层次患者的就医需求,有效规范医疗行为,控制医疗费用不合理的过快增长,是对解决"看病难、看病贵"问题进行的有益探索。Objective：To discuss the effect of the expandable service module of restrictive price of basic treatment of monopathy in standardi- zing medical behaviors and controlling the unreasonable increase of medical cost. Methods：According to the clinical pathways, the expandable service pattern of restrictive price of basic treatment of monpathy disease in three common diseases was implemented, and then the variation in the process of service was recorded, and finally the difference of two services was analyzed. Results： Compared to that of the corresponding time of the Year 2005, the total hospitalization fee, the ration of medicine cost and the mean hospitalization duration were obviously reduced but the number of operations increased significantly. Conclusion：The expandable service of restrictive price of basic treatment of monopathy can meet the medical requirements of different people, standardize effectively medical behaviors and control the unreasonable increase of medical cost. It may be a possible solution to the problem of “ It is so difficult and expensive to see a doctor”.
