非典型脑脓肿邻近脑膜异常强化的MR诊断

摘　　要：目的回顾性分析16例非典型脑脓肿临近脑膜异常强化的MR表现,探讨其诊断价值。资料与方法16例不典型脑脓肿均经手术病理证实,所有病人术前均行MR平扫及增强扫描。其中5例术前行CT平扫。结果单房性脑脓肿12例,多房性脑脓肿4例。其中脓肿腔>3cm者13例,有12例MR平扫可见脓肿壁呈中等信号"暗带",另4例中等信号脓肿壁显示不连续或模糊不清。16例增强后均呈环形强化,其中7例脓肿壁厚薄不均,5例脓腔形态不规则,4例在脓肿壁上可见结节状强化。16例增强后在脓肿邻近脑膜均见强化。结论认识不典型脑脓肿的MR表现特别是邻近脑膜强化对诊断很有价值。Objective Retrospectively analyze the MRI appearance of atypical brain abscess and investigate diagnosis value of neighboring meningeal enhanced of MRI in atypieal brain abscess. Materials and Methods The MRI findings of 16 eases with atypieal brain abscess proved by operation and pathology. All patients were performed by plain and eontrast MRI scanning. Plain CT sean was obtained in 5 eases before operation. Results 12 of 16 brain abseess lesions showed single eavity, 4 eases showes multiple eavity . The diameter of abscess eavity was more than 3era in 13 eases. On the wall of the brain abscess in 12 eases of routine MRI ,there was a ＂dark belt＂ with isointensity signal. Other 4 eases, the wall of the brain abscess is blur and ineontinous . All of 16 eases were rings enhanced, 7 eases showed the wall of abscess were uneven thickness, 5 eases presents irregular forms , 4 eases showed enhanced nodules on the wall of abscess. The abnormal enhancing meningeal neighboring the abcess were showed in all patients. Conclusion To master the MRI appearance of atypical brain abscess, especially the enhancing meningeal neighboring the lesion, is helpful for diagnosis.
