摘 要:目的探讨结缔组织生长因子(CTGF)在肾间质纤维化中的表达及温阳活血方的干预作用。方法采用单侧输尿管结扎(UUO)肾间质纤维化模型。将大鼠随机分为4组,即:假手术组、模型组、温阳活血治疗组、蒙诺组。药物治疗14天后,收集尿液,检测尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG),处死大鼠,腹主动脉采血观察肾功能,取肾组织观察其病理改变,并应用免疫组织化学方法检测肾组织CTGF的表达。结果模型组大鼠尿NAG,血尿素氮(BUN)较假手术组显著升高,温阳活血方组尿NAG和血BUN较模型组显著降低;模型组肾组织表现为纤维化病理改变,假手术组肾小管上皮细胞、肾间质细胞胞浆CTGF有较弱的表达;模型组呈强阳性表达;温阳活血组病理改变较模型组纤维化程度减轻,且CTGF的阳性表达较模型组显著减弱,差异有统计学意义(P<0.05),与蒙诺组比差异无统计学意义。结论温阳活血方可降低UUO大鼠尿NAG和血BUN,而具有保护肾小管功能和改善肾功能作用,可能与其能抑制肾组织CTGF过度表达而减缓肾纤维化病程进展。 Objective To survey the expression of connective tissue growth factor(CTGF) in renal interstitium fibrosis and the effect of Wenyang Houxue recipe.Methods Rats were ligated left ure-ter to make unilateral ureteral obstraction(UUO) animal model and randomized into four group:sham operated group,model group,Wenyang Houxue recipe treated group and fosinopril sodium treated group.By 14 day after treatment,N-acety1-β-D-glucosaminidase(NAG) of urine,renal function and renal pathological change were detected.Meanwhile,expression of CTGF was measured with immunohistochemistral method in renal tissue.Results The NAG and blood urea nitrogen(BUN) of model group was significantly higher than sham operated group.The NAG and BUN of Wenyang Houxue recipe treated group was significantly decreased than model group.Renal tissue of model group showed fibrous degeneration.The CTGF of sham operated group had lower expression in renal tubular epithelial cells and interstitial cell kytoplasm.However,the expression of CTGF was significantly increased in model group.Comparing Wenyang Huoxue recipe treated group with model group,the fibrosis degree was slighter and the CTGF expression decreased markedly in the former Both of the groups had statistics difference(P〈0.05).Conclusion Wenyang Huoxue recipe can reduce urine NAG and BUN of UUO model rats to protect renal tubule function and to improve renal function.The mechanism possibly inhibit the overexpression of CTGF in renal tissue to alleviate progression of renal fibrosis.
