摘 要:目的了解湖北省艾滋病抗病毒治疗病人的CD4细胞计数检测情况及在不同治疗时段的变化特点。方法利用国家艾滋病抗病毒治疗病人基本信息数据库以及实验室检测数据库,分析了湖北省2003年以来抗病毒治疗病人的CD4细胞计数检测情况,计算不同年份进入治疗的艾滋病病人在2003-2007年3月28日之间按照要求进行CD4随访检测率和开始抗病毒治疗后1年内按照要求随访检测率,并分析了不同治疗时段(治疗基线、治疗3、6、9、12以及18个月时)的CD4细胞计数变化特点。结果湖北省2003-2006年间抗病毒治疗人数累计达到1211人,累计CD4随访检测率呈上升趋势,治疗3、6、9、12以及18个月时,同治疗基线CD4相比,CD4细胞数明显增加,经统计学分析具有显著性差异。在治疗的头3个月内CD4细胞数平均增加29.21个/月,随着治疗时间的延长,CD4增幅减慢。结论艾滋病病人接受免费抗病毒治疗以后,CD4细胞计数逐渐上升,在治疗3个月时增幅最明显,此后缓慢上升。CD4细胞计数检测在艾滋病抗病毒治疗中发挥着越来越重要的作用。随着抗病毒治疗CD4随访检测比例不断上升,CD4检测试剂的需求逐年增加,应加强计划管理保证试剂的充足供应。Objective To investigate the current situation of CD4 cell count in AIDS patients after initiation of highly active antiretroviral treatment (HAART) in Hubei Province, and to study the dynamic changes at different stages of treatment. Methods Based on the data from the national antiretroviral treatment database and our laboratory testing database, the CD4 cell count testing among AIDS patients following antiretroviral treatment since 2003 in Hubei Province were analyzed. The reported rates that CD4 cell count was performed from 2003 to March, 2007 and within the first year after initiation of treatment were calculated. And the dynamic changes of CD4 cell count at different stages of treatment (baseline, 3, 6, 9, 12, and 18 months following antiretroviral treatment) were analyzed. A total of 1,211 AIDS patients received antiretroviral treatment from 2003 to 2006. There was a rising trend in cumulative CD4 cell count rates. Compared with the baseline values, the CD4 T lymphocyte at 3, 6, 9, 12, and 18 months following antiretroviral treatment increased significantly and the differences were statistically significant. This increase was particularly apparent in the first 3 months after treatment with 29.21/uL per month, and tended to slow over time. Conclusion With the availability of more CD4 testing instruments, the access to CD4 cell count is improved greatly. Following free antiretroviral treatment, CD4 cell count increases gradually. The CD4 T lymphocyte testing plays an increasingly important role in guiding the HAART. The reagent management for CD4 cell count needs to be better planned to ensure the sufficient supply.
