摘 要:目的评价头孢地嗪(CDZ)对新生儿败血症的临床疗效。方法采用临床病例对照观察研究方法,把112例新生儿败血症患儿随机分为CDZ组(43例)和对照(CG)组,CDZ组抗生素首选CDZ50mg·kg-1·d-1,q12h;CG组随机选用其他β-内酰胺类药物,选用复方抗生素者为CG1组(33例),选用单方者为CG2组(36例)。统计分析首选抗生素5d内换药率及各组间预后的差异。结果112例患儿首选抗生素共6种,5d内换药率CDZ组为12%,其他5种抗生素为36%~66%,首选抗生素的换药率和最终治疗的有效率呈负相关(r=-0.888,P<0.05)。三组有效率比较差异均有统计学意义(P值均<0.05)。结论CDZ临床疗效好,可作为治疗新生儿败血症的新选择。Objective To investigate the clinical effects of cefodizime (CDZ) on neonatal septicemia. Methods The cases of 112 neonates with septicemia were divided into 3 groups at random . The CDZ group with 43 cases received 50 mg/kg CDZ, q 12h. The control group (CG) received other β - lactam antibiotics at random, compound antibiotics for the CG1 with 33 cases and simple antibiotics for the CG2 with 36 cases. The changing ratio in 5 days of first choice antibiotics and the prognosis of 3 groups were analyzed. Results There were 6 antibiotics as the first choice in the 112 cases. The changing ratio in 5 days was 12% for CDZ group, and the others were 36% - 66%. The prognosis of neonatal septicemia had negative correlation with the antibiotics changing ratio in 5 days ( r = - 0. 888, P 〈 0. 05 ). There were statistical significance in the effective rates among the CDZ group ( 93% ) , CG1 group ( 73% ) and CG2 group(60% ) , all P 〈 0.05. Conclusion The C DZ has good clinical efficacy , and can be used as a new antibacterial choice for neonatal septicemia.
