膝关节镜下前、后交叉韧带同期重建的疗效观察

摘　　要：目的探讨采用关节镜下微创手术的方法同期重建膝关节前交叉韧带(ACL)与后交叉韧带(PCL)。方法l5例病人(15膝)经关节镜检查证实ACL和PCL均断裂,在膝关节镜下采用挤压螺钉固定同种异体跟腱、LARS人工韧带或自体双侧4股腘绳肌腱移植同期重建前、后前叉韧带。术后随访6-18个月,平均随访14个月。结果按Lysholm膝关节评分标准评分,平均积分由术前的24分提高到术后的85分,全组均未出现并发损伤。患者均在术后3-6个月恢复正常的工作及生活。结论关节镜下同时重建前、后交叉韧带手术创伤小,骨隧道定位准确,固定牢固,术后膝关节功能恢复满意。Objective To explore the technique of simultaneous reconstruction of anterior cruciate ligament （ACL） and posterior cruciate ligament （PCL） under arthroscope. Methods From June 2005 to Aug 2006, 15 knees with combined ACL and PCL tear in 15 patients were verified with arthroscopy. Simultaneous reconstruction of ACL and PCL under arthroscope with deep - frozen achilles tendon allograft, four - stranded hamstring tendon auto graft, Ligament Advanced Reinforcemement System （ LARS ） and extrusion - screw fixation. All of them were followed up from 6 to 18 months with an average of 14 months. Results According to the Lysholm score method, the average score was 24 before operation and the average score was 85 after operation. There was no severe complications occurred after operation in the 15 patients. All of them returned to normal within 3 to 6 months after operation. Conclusion It is effective method to simultaneously reconstruct ACL and PCL under arthrescope with minimal trauma in surgery and reliable satisfactory outcome.
