摘 要:目的从敏感性、特异性、早期和快速检测的角度探讨新的伤寒和副伤寒的实验诊断方法,以提高伤寒及副伤寒的实验室诊断水平。方法用间接免疫荧光法和肥达试验检测85例经血培养确诊的伤寒或副伤寒患者、20例献血员及65例非伤寒副伤寒发热病人血清伤寒及副伤寒IgM抗体,对检测结果进行统计学分析和处理。结果IFL检测85例伤寒或副伤寒患者血清IgM抗体,79例阳性,阳性率92.9%,病程6d以内,阳性率为90%;肥达试验检测同样病例,47例阳性,阳性率55.3%,病程6d以内阳性率46.7%,两种方法的检测结果具有显著性差异(P<0.01)。对献血员和其他发热病人检测,两种方法的阳性率均在2%以下,二者没有差异。结论检测抗伤寒和副伤寒沙门菌IgM抗体,免疫荧光法比肥达试验的灵敏度更高,免疫荧光法比肥达试验更具早期和快速检出的特点。Objective To investigate a rapid, sensitive and idio- laboratory diagnosis method to diagnose abdominal typhus and paratyphoid so that to enhance the level of laboratory diagnosis to this kind of disease. Methods Immunofluorescence (IFL) was used to detect the blood serum of 85 patients with abdominal typhus and paratyphoid ,20 people donated their blood and people with other disease for IgM antibody to Eberthella typhi and Bacillus paratyphosus, and the results were compared with that of Widal's test and analysed by the means of statistics. Results in immunofluorescence (IFL) detected group, the masculine ration of antibody was 92.9%,and that of Widal's test detected group was 55.3%. There are significant difference between the results of the two methods. Within 6 days, Their results was 90 VS 46.7%. Conclusions IFL is more sensitive than Widal's test when they are used to detect antibody and IFL can be used to diagnose abdominal typhus and paratyphoid rapidly and early.
