健胃愈疡颗粒对消化性溃疡患者胃黏膜IL8和TGFβ1表达的影响

摘　　要：目的:观察健胃愈疡颗粒(柴胡、党参、白芍、延胡索、白及、甘草等)对消化性溃疡患者胃黏膜IL8和TGFβ1表达的影响,探讨健胃愈疡颗粒的疗效机制。方法:56例胃溃疡(肝郁脾虚证)患者,随机分为治疗组和西药对照组,另设正常对照组;治疗组予健胃愈疡颗粒;对照组予法莫替丁片加硫糖铝片,4周为1个疗程,HP阳性者加用阿莫西林胶囊和甲硝唑片;治疗前后胃镜下取溃疡边缘黏膜活检。免疫组化及RT-PCR检测TGFβ、IL8表达水平;结果:(1)IL8在正常胃黏膜中呈阴性或偶有阳性表达,治疗前两组表达高于正常组(P<0.01),治疗后均减少至接近正常水平,两组无显著性差异(P>0.05)。(2)TGFβ1在正常胃黏膜中有少量表达,两组治疗前较正常胃黏膜增加,两组间差异无显著性(P>0.05);治疗后两组表达均显著增加,且治疗组高于对照组(P<0.05);结论:健胃愈疡颗粒可能通过抑制炎性细胞因子IL8的表达,增强促溃疡愈合因子TGFβ1的表达,达到促进溃疡愈合的目的。AIM： To explore the effects and its mechanism of Jianwei Yuyang Granule （JG） （Radix Bupleuri, Radix Codonopsis ,Radix Paeoniae alba ,Rhizoma corydalis, Rhizoma Bletillae ,Radix et Rhizoma Glycyrrhizae etc. ） on patient＇s gastric mucosa. METHODS： Fifty-six patients with gastric ulcer （GU） and Ganyupixu symdrome were di- vided randomly into two groups （JG-treated group and famotidine-and-sucralfate-treated group as control）. The ex- pression of IL 8 and TGFβ1 was measured with histochemistry and RT-PCR before or after treatment. RESULTS： The expression of II8 which was negative in normal mucosa increased before treatment and decreased after treatment （P 〈 0. 01 ）. There was no significant difference between JG-treated group and famotidine-and-sucralfate-treated group. The level of TGFβ1 which expressed weakly in normal mucosa, increased moderately in gastric ulcer mucosa and raised significantly after treatment. And there was remarkable difference between JG-treated group and famotidineand-sucralfate-treated group （P 〈 0.05）. CONCLUSION： JG can improve the quality of ulcer healing and accelerate the ulcer healing by suppressing the expression of IL8 and increasing the expression of TGFβ1.
