摘  要:目的探讨硫化氢(H2S)在儿童自主神经介导性晕厥中的变化。方法对2006年3月~2007年3月在北京大学第一医院儿科就诊的28例自主神经介导性晕厥患儿(男13例,女15例;年龄8~17岁,平均11.79岁)及82例健康正常儿童(男41例,女41例;年龄9~17岁,平均13.0岁)进行诊断学研究并应用敏感硫电极法测量血浆H2S水平。结果自主神经介导性晕厥患儿28例经直立倾斜试验检查后,10例血流动力学类型为体位性心动过速综合征,9例为血管迷走性晕厥血管抑制型,3例为血管迷走性晕厥心脏抑制型,5例为血管迷走性晕厥混合型,1例为直立性低血压。健康对照组儿童血浆H2S水平为(50.67±10.48)μmol/L,晕厥组为(24.74±4.57)μmol/L。二组比较有显著性差异(P=0)。不同血流动力学类型间H2S水平未见明显差异(P>0.05)。结论血浆H2S水平降低可能是儿童自主神经介导性晕厥发病机制之一。Objective To discuss the change of hydrogen sulfide( H2S)in plasma in children with neurally mediated syncope. Methods Data of 28 children ( 13 male, 15 female, aged 8 - 17 years, mean age 11.79 years) with unexplained syncope who came from the first hospital of peking university and 82 healthy children ( 41 males, 41 females, aged 9 - 17 years, mean age 13.0 years ) underwent diagnostic studies. Plasma H2 S level was detected by modified method with sulfide electrode. Results In 28 children with neurally mediated syncope, 10 were postural orthostatic tachycardia syndrome( POTS ) ,9 were vasovagal syncope (VVS) vasoinhibitory pattern ,3 were VVS cardioinhibitory pattern,5 were VVS mixed pattern and 1 was orthostatic hypotension(OH). The plasma H2 S levels were (24.74 ± 4.57 ) μ mol/L in syncopal cases and (50.67± 10.48 ) μmol/L in normal subjects (P = 0) ;the plasma H2 S level in syncope patients with different hemodynamic patterns did not have the trend of difference. Conclusion The decrease in plasma H2 S level may be one of the pathogenesis of neurally mediated syncope in children.
