摘　　要：目的探讨使用交锁髓内钉治疗股骨干大段粉碎性骨折的疗效。方法采用有限切开、不扩髓、尽可能少剥离骨膜、不强求解剖复位、静力固定的手术方法治疗22例股骨干大段粉碎性骨折。结果22例患者平均随访14个月,肢体关节功能恢复优良率90.9%(20/22),所有骨折均一期愈合,平均骨折临床愈合时间为6.4个月。结论使用交锁髓内钉是治疗股骨干大段粉碎性骨折的首选方法。应重视手术操作的细节处理,尽可能恢复肢体的长度及力线,尽可能减少对骨折端血供的侵扰,加强围手术期的护理及康复指导。Objective To investigate the clinical effects of inter-locking intramedullary nail in the treatment of multiple long comminuted fracture of femoral shaft. Methods The treatment of 22 adult patients of large comminuted femoral shaft fracture with limited open,unreamed marrow,minimal periosteal stripping,not forced anatomical re duction,static fixation was analyzed. Results 22 patients with an average of 14 months follow up,the rate of limb joints excellent functional recovery was 90. 9%（20/22）,all fractures healed,the average healing time was 6. 4 months. Conclusion Inter-locking intramedullary nail is the first choice in treating multiple long comminuted fracture. Attention should be paid to the details of the operation ,as far as possible to restore the length and power lines of limbs ;as far as possible to minimize the infestation to the blood supply of the fracture ;strengthen guidance to perioperative care and rehabilitation.
