实时三维彩色超声心动图诊断冠状动脉瘘的价值

摘　　要：目的：探讨实时三维彩色超声心动图在诊断冠状动脉瘘中的价值。方法：对15例先天性冠状动脉瘘患者行实时三维彩色超声心动图检查，实时立体显示扩张的冠状动脉并仔细追踪，寻找其在心腔的瘘口。结果：15例冠状动脉瘘中，右冠状动脉瘘6例，左冠状动脉瘘9例。二维超声和实时三维彩色超声心动图估测瘘口的最大径与造影估测大小的相关性分别为r=0．95，P〈0．05及r=0．99，P〈0．05。实时三维彩色超声心动图真实精确地显示了显著扩张的冠状动脉，以及瘘口处血流起源、方向和立体空间分布。结论：实时三维彩色超声心动图对冠状动脉瘘的诊断、治疗效果的评价有重要的临床价值。Objective To investigate the value of color real-time three-dimensional echocardiography （color RT 3DE） in patients with coronary artery fistula. Methods Fifteen patientsE9 male,6 female,mean age （38.5 ±11.3）years] with coronary artery fistula was diagnosed using Philips Sonos 7500 and IE33 with a X4 probe. Results Six patients were with right coronary artery fistulae （40~/oo） and nine patients with left coronary artery fistulae （60%）. The largest size＇of the fistulae measured by color two-dimensional echocardiography and color RT 3DE compared with those measured by coronary artery opacification, its correlation were separately r = 0.95, P 〈0.05 and r = 0.99, P 〈0.05, and color RT-3DE displayed clearly and accurately the dilated coronary artery, origin of blood flow, direction and its spatial relation to fistula. Conclusions Color RT-3DE has important clinical value on the diagnosis and the evaluation of therapeutic effect for coronary artery fistula.
