摘  要: 目的:探讨经胸超声心动图评价大鼠扩张型心肌病模型的价值。方法:取10周龄雄性SD大鼠40只。随机分为4组:对照组和阿霉素低、中、高剂量组,经腹腔注射阿霉素累计量分别为0、8、16、24mg/kg,于0、2、4、6、8周先后行超声心动图检查;第9周行血流动力学、心肌组织病理学检查。结果:超声心动图观察到随着阿霉素剂量的累积,大鼠左室舒张末径进行性增大,射血分数逐渐下降。8周时中剂量组左室舒张末径由(6.79±0.62)mm增加至(7.24±0.23)mm,射血分数由(92±2)%下降至(82±3)%(均P〈0.05);高剂量组左室舒张末径由(6.84±0.39)mm增加至(7.67±0.08)mm,射血分数由(92±2)%下降至(79±1)%(均P〈0.01)。血流动力学检查显示中、高剂量组左心室压峰值、左室压力最大上升及下降速度明显下降(均P〈0.01)。组织病理检查显示中、高剂量组心肌损伤严重。而对照组、阿霉素小剂量组上述指标均无显著改变。结论:经胸超声心动图可无创、准确、敏感地动态监测大鼠扩张型心肌病模型心腔及心功能的变化,有推广应用的价值。Objective To assess the value of echocardiography in evaluating rats with adriamycin(ADR)- induced dilated cardiomyopathy. Methods Forty male 10 weeks old SD rats were randomly divided in to 4 groups: control group,low dose ADR group, middle dose ADR group and high dose ADR group. Different accumulative total doses (0,8,16,24 mg/kg) of ADR were injected intraperitoneally to each rat in different group separately. The left ventricular dimension at end-diastole (LVDD) and ejection fraction (LVEF) were measured with echocardiography at baseline, 2nd,4th, 6th and 8th weeks after first injection. Haemodynamic and histopathological studies were done at the 9th week. Results The LVDD of rats in middle and high dose groups were progressively increased and LVEF were gradually reduced by the accumulation of ADR. At the 8th week,LVDD in middle dose group increased to (7.24 ± 0.23)mm from (6.79±0.62) mm( P 〈0.05) at the baseline,LVEF was decreased to (82 ± 3) % from (92 ±2) % ( P 〈0. 05) ; while LVDD in high dose group was increased to (7. 67 ± 0. 08) mm from (6.84±0.39)mm (P 〈0.01),LVEF was decreased to (79± 1)% from (92±2)%(P 〈0.01) in the same duration. Haemodynamic study showed in middle and high dose groups, the left ventricular systolic pressure, maximum left ventricular developed pressure increase rate and decrease rate were gradually reduced( P 〈0.01). Histopathological study showed serious cardiac muscle damage in middle and high dose groups. Such changes in histopathological,haemodynamic and echocardiography study were not be observed in control group and low dose group. Conclasions Echocardiography well monitor the changes of LVDD and LVEF of the rats with adriamycininduced dilated cardiomyopathy.
