摘  要:目的报道应用5块游离组织组合移植Ⅰ期修复全手皮肤撕脱伤的临床效果。方法采用同侧(足母)甲瓣与第二足趾移植再造拇指和示指,对侧第二足趾移植再造中指,双侧股前外侧皮瓣修复手背和手掌及虎口创面共6例。结果术后6例30块游离组织全部成活,受区皮瓣伤口Ⅰ期愈合2例,Ⅱ期愈合4例。随访时间1~9年,再造拇手指能对指对捏,生活能自理,手指两点辨别觉8~12mm,皮瓣感觉S_2。结论用5块游离组织组合移植修复手撕脱伤,虽然风险大,显微外科技术要求高,但术后功能恢复良好,是修复全手撕脱伤的最佳方法之一。Objective To report clinical effect about applying combined five free tissues transfer to repair the whole hand avulsion in one-stage. Methods Six cases were applied, all repaired in such a method: Wrap-round flap and the second toe in the same side of wound limb were used to repair thumb and index finger, the second toe in opposite side was used to reconstruct middle finger, bilateral anterolateral femoral flaps were used to cover the dorsal side and palmar side and the first web. Results All 30 tissues in 6 cases were survived, the recipient sites gained primary stage heal in 2 cases, 4 cases gained secondary stage heal. Followed -up 1 year to 9 years, all patients could care themselves in daily life, the reconstructed thumb and fingers could pinch, two-point discrimination was 8 mm to 12 mm, the sense of flap was S2. Conclusion Although with the high risk and high requirement of microsurgery technique in repair avulsion of whole hand with five free tissues, but the function is satisfactory. So it is the best procedure for repair avulsion of whole
