长期吸入糖皮质激素对哮喘患者肺功能的影响

摘　　要：目的探讨哮喘患者长期吸入糖皮质激素后临床症状评分、肺功能的动态变化。方法选择该科哮喘门诊1998年12月~2001年6月就诊患者共58例,按国家1997年哮喘防治指南进行分级治疗,动态观察其临床症状评分、肺功能。结果不管病情的轻重,患者吸入糖皮质激素治疗3个月后临床症状评分均显著减少（P〈0.01）,肺功能结果显示：中度组和重度组哮喘患者FEV1%在治疗3个月时显著增高（P〈0.05）,但以后又逐渐降低,治疗1a后均低于治疗3个月时水平（P〈0.01）。结论虽糖皮质激素吸入治疗可改善所有不同病情哮喘患者的临床症状,但只有尽早期应用糖皮质激素才能预防哮喘患者肺功能的降低。[Objective] To investigate the effect of long-term inhaled steroids （budesonide） on clinical symptoms and lung function of various asthmatics. [Methods] 58 patients with positive allergen skin test were included in this study. All patients received inhaled steroids（budesonide） according to guideline of GINA. At the time of enrolment and every 3 months all patients underwent pulmonary function test, estimated clinical manifestation and usage of β2- agonists. [Results] All patients symptoms were improved after inhaled budesonide. FEV 1% increased in moderate asthmatic group and sereve asthmatic group after 3 months inhaled budesonide （P 〈0.01）, however, it was decreasing after 3 months. [ Conclusions] These results suggest that ：the symptom of asthma can be improved in various asthmatics, however, only early intervention with steroids can prevent declining of pulmonary function in asthmatics.
