摘　　要：目的:探讨江汉油田慢性疾病发病状况与社区护理模式,使江汉油田在有限的条件内开展社区护理。方法:对3 098名石油工人进行慢性疾病基线调查后,对1 061名患有慢性疾病的患者在设置的护理工作模式下进行护理干预,并对干预效果进行评价。结果:进行护理干预后,慢性疾病患者对社区护理的认识和需求、防病保健意识、疾病的治疗效果、心理功能等有明显提高和改善,而且均明显优于护理干预前(P<0.01)同时降低了平均医疗费用。结论:社区护理模式适合油田矿区,在油田矿区开展社区护理是可行的,也是必要的。Objective ：To explored the situation of chronic disease community nursing model in Jianghan Oil Field to perform the community nursing in the limitcd conditions. Methods：3098 oil workers were basically surveyed for chronic disease, then the nursing intervention was conducted in the 1061 patients with chronic disease under the established community nursing model and the effects of intervention were evaluated. Results：The knowledge and needs of farmers with chronic disease towards community nursing ,awareness of preventing disease and health protection, the effects of treatment and psychological function were improved significantly （ P 〈 0.01 ） between before intervention and after intervention, and the acerage - year medical expense of the farmers was reducod. Conclusion：This community nursing model suits the features in Jianghan Oil Field, and it is practical and necessary to carry out the community nursing in Jianghan Oil Field.
