抗PR3抗体、抗MPO抗体检测的临床探究

摘　　要：[目的]探究抗蛋白酶3(PR3)抗体、抗髓过氧化物酶(MPO)抗体在系统性血管炎患者中检测的临床意义。[方法]应用酶联免疫吸附实验(ELISA)检测抗PR3抗体和抗MPO抗体,应用IIF法检测ANCA总抗体。[结果]105例系统性血管炎患者中57例韦格纳肉芽肿患者,主要表现为抗PR3抗体和颗粒型抗中性粒细胞胞质抗体(cANCA),阳性例数均为41例,阳性率均为71.93%,13例显微镜下多血管炎(MPA)和9例坏死性新月体肾炎(ATH)主要表现为MPO和pANCA,阳性率分别为53.8%和61.5%,22.22%和33.33%;135例系统性红斑狼疮(SLE)患者,主要表现为抗MPO抗体和核周型抗中性粒细胞胞质抗体(pANCA),阳性例数分别为29例和32例,阳性率分别是21.5%和23.7%。[结论]抗PR3、抗MPO抗体作为系统性血管炎的一种敏感标记抗体,有利于该疾病的早期诊断及治疗。[ Objective ] To study the clinical significance of anti - PR3 and anti - MPO＇s detection in systemic vasculitis. [ Methods ] Anti -PR3 and Anti- MPO antibodies were detected by ELISA, ANCA Was determined by indirect immunofluorescence （IIF）. [ Results ] We detected antibodies in 105 cases of systemic vasculitis . The positive rate of anti - PR3 antibody and cANCA was 71.93% （41/57） in 57 patients with Wegener＇s（WG）. The positive rate of anti - MPO antibody and pANCA was 53.8% and 61.5% in 13cases of systemic rasculitis under microscope （MPA） ,then this rate was 22.22% and 33.33% in 9 cases of ATH. The positive rate of anti - MPO antibody and pANCA was 21.5 % （29/135 ） and 27.3 % （ 32/135 ） in 135 cases of SLE. positive. [ Conclusion ] PR3 and MPO antibodies are sensitive markers of systemic vasculitis and they are useful for early treatment of the disease.
