摘 要:目的:测定男性2型糖尿病患者骨密度及各种骨代谢生化指标,探讨男性2型糖尿病1,25(OH)2D3患者骨质疏松的发病机理。方法:测定66例老年男性2型糖尿病患者及62例健康对照者的骨密度,血清骨钙素、睾酮、1,25(OH)2D3、25(OH)D3、碱性磷酸酶、血钙、磷、尿Ca/Cr、HOP/Cr、CTX/Cr等并进行比较。结果:男性2型糖尿病患者较健康对照组骨密度显著降低(P<0.01)。骨钙素、睾酮、1,25(OH)2D3、25(OH)D3浓度低于对照组(P<0.01);尿Ca/Cr、HOP/Cr、CTX/Cr显著高于对照组(P<0.01)。结论:男性2型糖尿病患者易并发骨质疏松,表现为骨吸收增加,骨形成下降。骨钙素、睾酮、1,25(OH)2D3等激素分泌及代谢失常,可能参与了骨质疏松的发生发展。Objective To investigate the change of bone mineral density and biochemical indicator of bone metabolism in male patients with type 2 diabetes mellitus and to analyze the possible pathogenesis. Methods Sixty-six male patients with type 2 diabetes mellitus and 62 healthy control were selected to determine the bone density, bone gla protein, testosterone, 1,25(OH)2D3, 25(OH) D3, alkaline phosphatase, blood calcium, phosphonium, urine calcium, urinary hydroxyproline and CTX in all cases. Results The bone mineral density of type 2 diabetes mellitus patients were lower than normal controls (P〈0.01). Bone glaprotein testosterone, 1,25 (OH)2D3 and 25 (OH)D3 in type 2 diabetes mellitus group were significantly lower than the normal controls(P〈0.01). The urine Ca/ Cr, urinary hydroxyproline /Cr and CTX/Cr in type 2 diabetes mellitus group were significantly higher than the normal controls(P〈0.01). Conclusion There is a higher morbidity of osteoporosis in male type 2 diabetes mellitus patients. Bone formation is decreased and bone absorption is increased. The mechanism of osteoporosis includes the upset of hormone secretion and metabolism such as bone gla protein, 1,25(OH)2D and T.
