单纯胸腰椎骨折早期中医证候分布特征的初步研究

摘　　要：目的：研究单纯胸腰椎骨折患者早期症状分布特点,归纳单纯胸腰椎骨折常见的中医证候类型及其诊断要点。方法：对广东省中医院2002~2006年近5年确诊为单纯胸腰椎骨折的病例采用回顾性病历研究工作,收集相关的证候学资料建立SPSS数据库,用频数分析统计中医证型和辨证要点。结果：单纯胸腰椎骨折的主要症状和体征按频次排列主要为：疼痛,舌暗,脉弦,便秘,舌红,脉细,腹胀,纳差。主要辨证分型是：肝肾不足、气滞血瘀型,以及气滞血瘀型。与单纯胸腰椎骨折各辨椎骨折各辨证要素密节相关的临床症征：气滞血瘀要素：疼痛、便秘、腹胀、舌暗与脉弦;肝肾不足要素：年龄≥50岁,（腰脊部）疼痛、脉弦、脉细、头晕、耳鸣;热要素：便秘、口干苦、舌干、舌红、苔黄、苔黑、脉数;湿要素：便秘、腹胀、纳差、脉弦、舌红、脉细、苔黄、苔腻、口干苦;气阴两虚要素：便秘、腹胀、纳差、脉细、脉弱、舌红、口干苦、苔干、眠差、苔少、脉数。结论：单纯胸腰椎骨折早期主要症状是疼痛,舌暗,脉弦,常见的中医证候类型是肝肾不足、气滞血瘀型以及气滞血瘀型。Objective：To study the distributive characteristic of symptoms about simple thoracolumbar and to summarize the differentiation of these syndromes. Methods： Select Guangdong provincial hospital of TCM, considering study the cases history of simple thoracolumbar fractures from 2002 to 2006, apply Frequencies to analyze the type of syndrome and signs for classlfcation of syndromes. Result： From high to low frequencies, the main symptoms of simple thoracolumbar fractures are pain, dull tongue, stringy pulse, constipation, red tongue, thready pulse, stomach symptoms,poor sppetite. From high to low frequencies, the main Chinese medicine differentiation sub-standard are weakness of liver and kidney and blood stasis and qi sluggish, blood stasis and qi sluggish followed. All types of closely related elements of the clinical disease symptoms：Blood stasis and ql sluggish：pain, constipation, stomach symptoms, dull tongue and stringy pulse. Weakness of liver and kidney：over 50, （back） pain, stringy pulse, thready pulse, dizziness and tinnitus. Hot： constipation, dry mouth and bitter taste, dry rut, red tongue, yellow fur, black fur, fast pulse. Wet ： constipation, stomach symptoms, poor appetite, stringy pulse, red tongue, thready pulse, yellow fur, greasy fur, dry mouth and bitter taste. Weakness of yin and qi：constlpation, stomach symptoms, poor appetite, thread pulse, weak pulse, red tongue, dry mouth and bitter taste, dry fur, poor sleep, uncoated tongue surface, fast pulse. Conclusion： Main symptoms of simple thoracolumbar fractures are pain, dull tongue and stringy pulse. The mainest Chinese medicine differentiation sub-standard is weakness of liver and kidney and blood stasis and qi sluggish, then blood stasis and qi sluggish followed.
