复合自动加压式外固定架的研制及其在跟骨粉碎性骨折中的应用

摘  要:目的研制一种自动加压式外固定装置,用于治疗跟骨的粉碎性骨折,为临床处理这类骨折,提供一种安全、简单、有效的治疗方法。方法此装置用医用不锈钢材料,配合克氏针(斯氏针),通过稳定前足和胫骨使复位的跟骨得以固定牢固。结果5例跟骨粉碎性骨折用此架治疗,术后依据Maryland足部评分系统,优4例,良1例,无针道感染、踝关节功能障碍、跟骨痛等并发症发生。结论复合自动加压式外固定架治疗跟骨骨折,方法操作简单,跟骨塑性好,关节面复位满意,成本低。其器械原理、结构设计和治疗方法可行,值得普及和推广。Objective To develop a kind of compound follow up pressure type external fixing apparatus in treatment of comminuted fracture of calcaneus and provide a safe, simple and effective therapeutic method. Methods This apparatus, made of special stainless steel for medical use, combined with Kirschner wire, fastened displaced calcaneus by fixing forefoot and shinbone. Results Five patients were treated by this apparatus and the curative effects were assessed by Maryland Foot Part Assessment System. According to the as- sessed results, four patients were excellent and one patient was good. There were no complications such as wire channel infection, dys- function of ankle joint and calcaneus ache. Conclusion The compound follow up pressure type external fixing apparatus has advantages in treating comminuted fracture of calcaneus with good moulding of calcaneus, satisfactory reduction of articular surface and low cost. The apparatus principle, structure design and operating procedure of this apparatus are available. This apparatus and relative technique are good for popularizing.
