摘  要:目的研究肝移植术后静脉应用前列地尔(PGE_1)对患者血清内皮素和肾脏血流的影响。方法将肝移植术后的患者分为治疗组(38例)及对照组(18例)。治疗组术后持续静脉泵入PGE_1,连续应用10d,对照组不应用PGE_1。检测围手术期两组患者的血清内皮素含量及肾脏血流指数。结果肝移植术前和术后当天治疗组和对照组血清内皮素含量均高于正常,但两组问差异无统计学意义(P>0.05);术后10d时,两组的血清内皮素均显著下降,但治疗组明显低于对照组(P<0.01)。对照组和治疗组术前及术后当天肾脏血管阻力指数均高于正常值,两组间差异无统计学意义(P>0.05);术后10d时肾脏血管阻力指数下降,而治疗组要明显低于对照组(P<0.01)。术后10d时治疗组血清肌酐也明显低于对照组(P<0.01)。结论肝移植术后应用PGE_1能够明显改善肾功能;其主要作用机制为降低血清内皮素含量,扩张肾脏血管,增加其有效灌注量。Objective To assess the effect of prostaglandin E1 (PGE1) on renal blood flow and serum endothelin of liver recipients. Methods PGEx was administered in 38 liver recipients at the dose of 0. 6μg.kg^-1 · h^-1 during liver transplantation and every day after operation. The effects of PGE1 on serum endothelin concentration and creatinine (Cr) were observed and these indexes were compared with those in the control group (n = 18). The renal blood flow resistance indexes (RI) were measured by Doppler ultrasound. Results Cr and RI were significantly lower in PGE1-treated group than those in the control group. PGE1-treated group also showed a significantly lower serum endothelin concentration. Conclusion Administration of PGE1 in liver recipients can significantly improve the early renal function by reducing serum endothelin concentration and dilating renal blood vessels.
