摘  要:目的研究核潜艇长航对艇员外周血红细胞系统的影响,并分析产生影响的可能原因。方法(1)用血液分析仪检测RBC、HGB、HCT、平均红细胞体积(MCV)、平均红细胞血红蛋白(MCH)、平均红细胞血红蛋白浓度(MCHC)、红细胞分布宽度(RDW-SD)。(2)采用煌焦油蓝活体染色法检测网织红细胞(RET)数。(3)用联苯胺法测定血浆游离血红蛋白;用半定量法测定红细胞的渗透脆性;用全自动生化分析仪测定血清间接胆红素。观察核潜艇艇员长航40 d后各指标的变化。结果(1)长航后RBC、HGB、HCT、MCV分别为(5.15±0.39)×1012.L-1、(160.4±12.4)g.L-1、(45.9±4.26)%、(91.4±2.93)fl,均明显高于长航前的检测值〔(4.86±0.28)×1012.L-1、(152.9±7.61)g.L-1、(44.3±2.07)%、(88.7±2.75)fl,均P<0.01)〕。(2)长航后RET(0.024±0.010)明显高于长航前(0.016±0.011,P<0.01)。(3)长航后红细胞的渗透脆性完全溶血的NaCl浓度、游离血红蛋白浓度、间接胆红素浓度分别为(0.37±0.21)g.mL-1、(167.1±40.8)mg.L-1、(16.59±7.44)μmo.lL-1,均明显高于长航前的检测值〔(0.33±0.25)g.mL-1、(51.2±17.2)mg.L-1、(12.02±4.56)μmol.L-1均P<0.01〕。结论核潜艇长航明显影响艇员外周血红细胞系统,可导致艇员RBC、HGB、HCT、MCV明显升高,RET、游离血红蛋白和血清间接胆红素明显升高,红细胞脆性加重。Objective To study the effect of long term voyage on red blood cell system of nuclear- powered submariners. Methods (1) RBC component was tested by blood analyzer. (2)reticulocyte (RET) count was tested by staining with dye. (3) RBC osmotic fragility was tested by semi - quantitative method; free plasma HGB was tested by benzidine and the indirect bilirubin by biochemical auto - analyzer. Results ( 1 ) After long term voyage, the level of RBC[ (5.15 ± 0.39)× 10^12·L^-1 ), HGB[ ( 160.4 ±12.4) g· L^-1 ), HCT[ (45.9 ± 4.26) %), MCV[ (91.4 ±2.93) β were higher than those before[(4.86 ± 0.28) × 10^12·L^-1, (152.9±7.61)g·L^-1, (44.3 ±2.07)%, (88.7 ±2.75) %.P 〈 0. 013 respectively. (2) RET count (0. 024 ± 0. 010) was higher than before (0. 016 ± 0. 011, P 〈 0.01 ). (3) RBC osmotic fragility [ (0.37± 0.21 ) g·mL^-1], free HGB [ ( 167.1 ± 40.8) mg·L^-1 ], and indirect bilirubin [ ( 16.59 ±7.44) μmnol·L^-1 ] in nuclear-powered submariner were also higher than before [ (0.33 ± 0.25) g· L^-1, (51.2 ± 17.2) mg· L^-1, ( 12.02 ± 4.56)μmmol· L^-1, P 〈 0.013. Conclusion Long term voyage may affect RBC system in nuclear- powered submaritr leadimg to increase in RBC, HGB, MCV, RET, plasma free HGB, indirect bilirubin, and aggravation of RBC osmotic fragility of nuclear- powered submariners.
