64层容积CT对主动脉夹层的术前诊断及介入术后疗效评估的价值

摘　　要：目的:评价64层容积CT对主动脉夹层术前诊断及介入术后评估的价值。方法:对由彩超提示的8例主动脉夹层DebakeyⅢ患者,分别于介入术前、术后行彩超、数字减影X线造影(DSA)和64层容积CT扫描,对比分析3种方法对主动脉夹层的诊断价值。结果:彩超、64层容积CT、DSA均对8名患者做出主动脉夹层的诊断和正确分型;3种方法均能清晰显示主动脉腔内撕裂的内膜片。64层容积CT共显示8例患者破口29个,覆膜支架破口封堵术后内漏2个;DSA显示破口20个,介入术后内漏1个;彩超提示10个破口,显示术后内漏1例。结论:64层容积CT对主动脉夹层的诊断及介入术前、术后评估的价值优于DSA和彩超。Aim： To appraise the value of 64 slices spiral CT in the diagnosis and treatment of aortic dissection. Methods：Eight patients with aortic dissection underwent 64 slices spiral CT angiography pre- and post- operation of intervention therapy, and compared with Doppler and digital subtraction angiography （ DSA）. ReSults： Three methods all made the correct diagnosis, classification and manifest torn septa in all cases. But 29 breaches were showed by 64 slices CT in 8 cases, Doppler and DSA were 10 and 20, respectively. 64 slices CT found 2 leakages after transluminal stent-graft placement and DSA 1, Doppler 1. Conclusion ： 64 slices spiral CT angiography is an excellent imaging modality for comprehensive evaluation of aortic dissection, and has more advantages compared with DSA and Doppler.
