摘 要:目的探讨彩超多谱勒超声(CDFI)在诊断恶性淋巴结肿大中的价值。方法用CDFI检测138例恶性肿瘤患者(恶性淋巴瘤42例、乳腺癌21例、肺癌45例、肝癌6例、肾癌5例、鼻咽癌5例、胃癌6例、食道癌5例、恶性黑色素瘤3例)的淋巴结,观察其回声强度及血供情况。结果所有的淋巴结呈现低回声;血供情况:恶性淋巴瘤80.95%(34/42)血流丰富、19.05%(8/42)血流少,乳腺癌33.33%(7/21)血流丰富、66.67%(14/21)血流少,肺癌31.11%(14/45)血流丰富、68.99%(31/45)血流少,肝癌83.33%(5/6)血流丰富、17.67%(1/6)血流少,肾癌80%(4/5)血流丰富、20%(1/5)血流少,鼻咽癌80%(4/5)血流丰富、20%(1/5)血流少,胃癌66.67%(4/6)血流丰富、33.33%(2/6)血流少,食道癌20%(1/5)血流丰富、80%((4/5)血流少,恶性黑色素瘤33.33%(1/3)血流丰富、66.67%(2/3)血流少。结论超多谱勒技术(CDFI)分析肿大淋巴结的回声强度及血供情况,有助于恶性淋巴结的诊断及其原发病灶的判断。Objective The study was designed to evaluate the clinical function of using color Doppler ultrasonography (CDFI) in diagnosing malignant lymphadenosis. Methods Using CDFI to observe the echo and bloodstream signal in 138 patient's lymphadenosls, consisting of 42 cases with lymphoma, 21 cases with breast cancer, 45 cases with lung cancer, 6 cases with hepatoma, 5 cases with renal cell carcinoma, 5 cases nasopharyngeal carcinoma, 6 cases with gastric carcinoma, 5 cases with esophageal Cancer and 3 cases with melanoma. Results Poor echo were seen in all of patient's lympho modes. The situation of bloodstream was as followings: lymphoma: 80.95%(34/42) appeared strong bloodstream signal and 19.05%(8/42) weak bloodstream signal; breast cancer. 33.33%(7/21) appeared strong bloodstream signal and 66.67%(14/21) weak bloodstream signal; lung cancer 31.11%(14/45) appeared strong bloodstream signal and 68.99%(31/45) weak bloodstream signal; hepatoma; 83.33%(5/6) appeared strong bloodstream signal and 17.67%(1/6) weak bloodstream signal; renal cell carcinoma: 80%(4/5)appeared strong bloodstream signal and 20%(1/5) weak bloodstream signal; nasopharyngeal carcinoma: 80%(4/5) appeared strong bloodstream signal and 20%(1/5) weak bloodstream signal; gastric carcinoma:66.67%(4/ 6) appeared strong bloodstream signal and 33.33%(2/6) weak bloodstream signal; esophageal Cancer: 20%(1/5) appeared strong bloodstream signal and 80%(4/5) weak bloodstream signal; melanoma: 33.33%(1/3) appeared strong bloodstream signal and 66.67%(2/3) weak bloodstream signal. Conclusions CDFI is valuable in diagnosing malignant lymphadenosis via observing echo and bloodstream signal.
