进行性皮质下动脉硬化性脑病临床分析及全程干预护理

摘　　要：目的：探讨进行性皮质下动脉硬化性脑病（Binswanger disease,BD）患者的认知功能障碍及其他特点,寻找有效的护理干预模式。方法：采用精神状态简易速查表（MMSE）、临床记忆量表（CMS）、日常生活能力量表（ADL）及Hamilton抑郁量表（HAMD）对35例BD患者（BD组）及32例健康老年人（对照组）进行测评。将确诊患有痴呆的BD患者随机分为全程护理干预组和住院护理指导组,前者采用住院-社区-家庭全程护理干预,后者实施一般住院护理指导。结果：BD患者的MMSE及CMS评分均明显低于健康对照组（P〈0.01）,ADL及HAMD评分均明显高于健康对照组（P〈0.01）。BD患者中全程护理干预组干预后MMSE及ADL评分均优于住院护理指导组（P〈0.05）。结论：BD患者认知功能损害严重,日常生活能力较差,且伴有不同程度的抑郁症状,全程护理干预可改善患者智能和日常生活能力,延缓病情进展。Objective To explore the cognitive function and other characteristics of Binswanger＇s Disease and seek for the efficient nursing interventions. Methods The mini--mental--status examina- tion （MMSE）, the Clinic--memory--status examination （CMS）, the activities of daily living measuring scale （ADL） and Hamilton depression scale （HAMD） were performed for 35 patients with Binswanger＇ s disease and 32 persons in healthy control group. The data was analyzed and compared. Patients with dementia were randomly divided into the whole--course nursing intervention group and the in--hospital nursing group. Results The scores of MMSE and CMS of Binswanger＇s disease group were significantly lower than that in healthy control group （P 〈0.01）, the scores of ADL and HAMD were significantly higher than that in healthy control group （P 〈0.01）. After the nursing intervention, MMSE score was significantly improved and ADL score was significantly decreased in the whole--course nursing group, compared with in--hospital nursing group （P 〈0.05）. Conclusions Patients with Binswanger＇s disease have severe cognitive impairment, worse activities of daily living, and depression symptoms, The whole course nursing intervention efficiently improves the patients＇ activity of daily living and retards the progress of the patients＇ disease.
