摘 要:目的研究乌司他丁对大鼠肝脏缺血再灌注后多器官损伤的保护作用。方法随机将Wistar大鼠分成假手术组(S组)、缺血再灌注组(I/R组)和乌司他丁保护组(U组)。采用阻断肝动脉、门静脉30min,恢复血流行再灌注制作肝热缺血动物模型。各组于再灌注1.5h、3h、6h和12h分别采血,测定DAO、ALT、AST、BUN和Cr水平。同步切取肝、肾、肺、小肠,切片后行苏木素-伊红染色(HE)比较病理形态学差异。TUNEL法测定细胞凋亡,免疫组化法测定Caspase-3和NF-κB表达,进行量化分析。结果I/R组与S组比较,DAO、AST、ALT、BUN、Cr水平明显升高(P<0.05);凋亡指数明显升高(P<0.05);肝、肾、肺、小肠病理损伤严重;Caspase-3和NF-κB表达也相应增加(P<0.05)。U组与I/R组比较,各项指标均明显降低(P<0.05)。结论乌司他丁对大鼠肝脏缺血再灌注后多器官损伤具有明显的保护作用。Objective To study protective effects of ulinastatin (UTI) on multiple organ injury following hepatic ischemia-reperfusion (I/R) in rats. Methods Male Wistar rats were randomly assigned into Sham, I/R, and UTI groups.Thirty min before operation, UTI(2×10^5U/L, 1 .5mL/kg) was given via back vein of the rats' penis. Hepatic ischemia was produced by occlusion of hepatic artery, 30min later, the blood flow was recovered by removing the clamps. The serum levels of DAO, ALT, AST, BUN, and Cr were measured at 1.5h, 3h, 6h, and 12h after reperfusion. Tissues from livers, kidneys, lungs and intestines were sectioned and stained with hematoxylin and eosin to observe the changes of histological pathology. TUNEL was performed to examine apoptotic cells. The expressions of Caspase-3 and NF-κB were examined by immunohistochemistry. Results The serum levels of DAO, ALT, AST, BUN, and Cr in I/R group were significantly higher than those in Sham group (P 〈 0.05). Histological staining showed serious injury in livers, kidneys, lungs, and intestines in I/R groups. More TUNEL-positive cells were observed in I/R group( P 〈 0.05). Caspase-3 and NF-κB expression levels also increased significantly in I/R group. However, all parmneters were decreased in UTI group( P 〈 0.05). Conclusion UTI has protective effects on multiple organs injury following hepatic ischemia-reperfusion in rats.
