血浆BH4水平与2型糖尿病大血管并发症的相关性研究

摘　　要：目的研究血浆四氢生物蝶呤(tetrahydrobiopterin,BH4)与2型糖尿病大血管并发症的关系。方法100例T2DM患者分为两组:未合并大血管并发症组41例,合并大血管并发症组59例。并设正常对照25例,检测血浆BH4及其他相关指标。结果(1)2型糖尿病组BH4水平明显低于对照组(P<0.01);糖尿病合并大血管病变组血浆BH4水平明显低于未合并大血管病变组(P<0.01)和正常对照组(P<0.01);(2)随着HBAlc水平的增高,血浆BH4水平呈下降趋势;(3)回归分析显示BH4水平与TC呈显著负相关(P<0.01),与CRP呈负相关(P<0.05)。结论血浆BH4水平在2型糖尿病患者中明显降低,合并大血管病变者降低更为明显,提示血浆BH4水平降低是2型糖尿病大血管病变的危险因素。Objective To investigate the relationship between the levels of plasma tetrahydrobiopterin （BH4） and type 2 diabetes mellitus, its macrovascular complications. Methods Forty-one cases of type 2 diabetic patients without macrovascular complications （ SDM group）, fifty-nine cases of type 2 diabetic-patients with macrovascular complications （T2DM ＋ MCV group）. Their BH4 and other indexes were measured. Twenty-five healthy subjects were served as a control group. Results： The plasma BH4 levels were significantly decreased in T2DM group compared with control group （P 〈 0.01 ）. The plasma BH4 levels in the T2DM ＋ MCV group were decreased compared with that in SDM group and that in the control group （P 〈0.01 ）. The difference of the plasma BH4 levels between control group and SDM group was significant （P 〈 0.01 ）. Increasing as the HBAlc concentration, there is a decreasing tendency of the plasma BH4 level. Multiple regression analysis revealed that the plasma BH4 level was correlated negatively with to- tal cholesterol and C- reactive protein. Conclusions The levels of plasma BH4 in type 2 diabetic patients were significantly decreased. The levels of plasma BH4 were decreased in T2DM ＋ MCV group compared with that in SDM group. The results suggest that the decreasing BH4 is a predictor of the incident of macrovascular complications in T2DM.
