实性肿块型肺隔离症的CT诊断

摘　　要：目的:探讨实性肿块型肺隔离症CT表现特点,以提高其诊断率。方法:回顾性分析9例经手术证实的实性肿块型肺隔离症患者的CT资料,观察病变的部位、形态、密度、边缘、与周围结构关系及其强化特点。结果:本组病例位于两肺下叶后基底段8例,左肺前内基底段1例;病变形态呈珊瑚状7例;密度不均匀9例,表现为多数囊样稍低密度区,强化后尤为明显;边缘光滑清楚者8例,边缘模糊者1例;病灶与主动脉分界清楚9例,相邻横膈面胸膜肥厚7例,形状呈肋骨样肥厚,;动脉期病灶内可见多数扭曲紊乱的血管影者9例,可见供血动脉起始部者2例,肿块边缘及主动脉旁见圆形血管强化断面影9例。结论:实性肿块型肺隔离症在发生部位、形态、密度、边缘、与周围结构关系及其强化特点具有一定的特征性,通过对其CT表现全面分析能够提高实性肿块型肺隔离症的术前诊断的正确率。Objective：To discuss CT appearances of soft-tissue mass pulmonary sequestration （PS）, and to improve the diagnostic aecnracy. Methods： The CT images of all 9 cases of surgery proved soft-tissue lump PS were retrospectively analyzed, and observed the locum, the morphous, the density, the margin, ambient structural relationship, and the feature of enhancement. Results：Among the 9 cases, 8 eases lesions located in posterior basal segment of lower lobe of lung, 1 case lesions in segmentum hasah antefius and medisd basal segment; With respect to the morphous characters, 7 cases were coral-like; 9 cases were inhomogeneous in density with the more low density areas of balloon, and were obviously after postcontrast CT; 8 cases had clear margin, and the other one had unclear margin; 9 cases had clear demarcation with artefiae aorta, the neighboring diaphragm pleural membrane of 7 cases were phmp fib-like; the more indiscriminate blood vesels were found in lump on arteral phase, and 2 cases were found initiation of feeding artery; 9 cases were found blood vessels enhanced by the lump or the arteriae aorta. Conclusion：There are some relatively specific features of soft-tissue lump PS in the locum, the morphous, the density, the margin, ambient structural relationship, and the feature of enhancement, which may improve the diagnostic aecnracy in preoperative diagnosis.
