摘　　要：[目的]为掌握永州市各级医疗机构法定传染病疫情报告情况,及时发现问题,以便进一步提高报告质量和减少漏报发生。[方法]2006年12月,对全市辖区内县级以上实行网络直报的医疗机构及随机抽取每县1/4以上的乡镇卫生院调查2006年1～9月诊疗的法定传染病,通过查对传染病登记本、报告卡和网络报告,判断传染病疫情报告情况。[结果]合计调查90家医疗机构,查出法定传染病25种5049例。漏报1022例,漏报率为20.24%。市、县、乡三级医疗机构法定传染病漏报率分别为0.80%、21.25%、65.40%,不同级别医疗机构漏报率的差异有统计学意义(P<0.01)。医疗机构各部门中,急诊科、化验室、门诊部、住院部的漏报率分别为40.00%、30.53%、25.60%和7.63%,差异有统计学意义(P<0.01);漏报病种居前5位的依次为痢疾、病毒性肝炎、肺结核、伤寒、麻疹,漏报率分别为17.73%、9.44%、23.11%、50.00%和7.88%。有36.76%的医疗机构漏报率低于20.00%,19.12%的医疗机构无传染病登记报告。[结论]永州市医疗机构法定传染病疫情报告工作较差,以乡镇卫生院和急诊科、化验室为主,应采取针对性措施降低法定传染病的漏报率。[Objective]To understand the reporting status of notifiable diseases in medical institutes at different levels , find problems , improve the quality , reduce missing report. [Methods]Medlcal institutes above county level , which had direct net reporting system and more than 1/4 town clinics were randomly selected in December 2006. The reporting status were tested through checking the record books of infectious.diseases, report cards from January to September 2006. [Results]90 medical institutes were investigated, there were 5049 cases with 25 different infectious diseases, 1022 cases were missed to report, accounting for 20.24%. The missing report was 0.80% ,21.25% ,65.40% in medical institutes at city, county and town levels. There was a difference （ P 〈0. 01）. The missing report in Emergency Department, Laboratory , Out-patient Department , In-patient Department was 40.00 %, 30.53%,25.60% and 7.63% respectively. There was a difference（ P 〈0.01）. The missing report of dysentery, viral hepatitis, pulmonary tuberculosis, typhoid fever and measles accounted for 17.73 %, 9.44 %, 23.11 % ,50.00% and 7.88% respectively , ranking the first five. The missing report was below 20% in 36.76% of the medical institutes. There was no record book for infectious diseases in 19.12% of the medical institutes. [Conclusion]The status of the report of infectious diseases were bad in Yongzhou, effective measures must be taken , especially for town clinics,emergency room and laboratory to reduce the missing report.
